He could lie and say that he isn't afraid, but in his cell there was no one to lie to only himself.
He can't convince himself he wasn't afraid. It's his own fault that he ended up in Azkaban, he won't blame it anyone else, but himself. He deserves to be here and fear he feels, the terror he feels is everything he inflicted on people in the past.
He does actually regret the things he did, but there is no one to tell and even there wasn't they wouldn't forgive him for things he done.
So here he was the once proud Pureblood in Azkaban paying for his sins that would never be forgiven.
