Name of deceased: James Tiberius Kirk

Name of Starship: Enterprise

Incident: On star date 11 April 2261 Captain James T. Kirk, First Officer Commander Spock and Ensign Pavel Chekov land on Class-M planet Xula 8976. Shortly after landing, a life force was discovered on the eastern side of the planet and the landing party went to investigate. The life force found was an approximately 86 feet dinosaur like creature. Upon, feeling the presence of the landing party the creature rushed to attack though no attack had been provoked.

The landing party withdrew their phasers to protect themselves. The creature, the kicked the Ensign and raised its left front foot to trample the captain. The captain ducked and ran behind the creature while the First Officer engaged it in battle. The creature suddenly reveled its tail which then coiled itself around the captain and and threw im under one of its feet which then crushed the captain. The Ensign, who had recovered by then alerted the Enterprise which beamed all three of them up. The captain was immediately brought into surgery while the First Officer and Ensign were treated for their injuries and both required minor surgeries.

The Chief medical Officer of the Enterprise, Dr. Leonard Horatio McCoy operated on the Captain for nine hours. During the last hour of the surgery the Captain went into cardiac arrest and time of death was 3:46 am, in the morning, Earth time while following a 12-hour clock.
