Janeway is seeing herself in Indiana, on the beach. Chakotay is talking to her, leading her through meeting her animal guide.
Chakotay: Do you see an animal?
Janeway: Actually…I see…two?
Chakotay: That is odd but not unheard of. The two animals are fighting between who will be your guide. It has been known to happen with extraordinarily strong people, sound like anyone? He chuckles to himself a little. Walk over to them, reach out your hand towards them, talk to them. The one that will become your guide will come to you.
Just then something happened. A wave of radiation passed through the whole ship. During that time a rattle snake slithered up onto Kathryn's arm. It wraps itself around her arm. It then bites her.
Janeway (still in an "unconscious" state): Ahhh!
Chakotay: What? Kathryn, what is it?!
Janeway: The…it bit….she then to Chakotay's horror slumps to the ground passed out, her hand falls out of the akunah. He catches her head just as it hits the ground. He taps his comm. badge.
Chakotay: Commander Chakotay to Sickbay , medical emergency in the Captain's ready room. Two to beam in.
Janeway and Chakotay are beamed into the sickbay. He places her in the biobed. The Doctor just materialized.
Doctor: Please state the nature of the…oh! What happened to her? He runs over to the captain and scans her.
Chakotay: I was helping her make contact with her animal guide. She said something about being bit. She then passed out. How is she?
Doctor: Commander I don't have good news. The Captain is in a coma.
