Smoking: pt ceased 3/12, further cessation advice given.

MW: nil
MH: hist of narcotic abuse, allergy penicillin, hist mi.
Sugar intake erratic.
Rads: LBW diag Ulq, shadow apical UL&.
TCA 2/52, RCT, C/prep, S+P.
