They aren't really identical.
They are closer to mirror images. Hikaru is not Kaoru, Kaoru is not Hikaru, as strange and unnatural that may sound. Hikaru is Kaoru turned around, and Kaoru is Hikaru in the mirror. When you line something and it's mirror image next to each other, there's just something off about it- They are flipped, but not opposite.
That's how Haruhi could tell them apart from the start- They were twins, but not identical ones. She imagines talking to her image, holding hands with a flipped version of herself, and feels infinitely strange, thinking about a relationship like theirs.
Perhaps it's some strange fetish, or just narcissism on their parts.
