Wally was fighting Captain Cold with Flash after Captain Cold tried to rob a bank. Key word tried. Kid Flash and Flash intervened and were currently avoiding numerous blasts from Captain Cold's ice gun. But KF didn't see one coming and his head was suddenly encased in a brick of ice. Flash quickly wrapped up the fight and went to his nephew who had only a few bits of frost left on his face.

What Barry didn't know was that the ice had gone on his eyes and before it could all be melted away, the cold had shut his eyes down and permanently blinded Wally (ok I have no idea if that would actually work or not). "I'm going to the mountain," Wally said, Flash went home and Wally warily made his way to the Zeta beam, it was lucky he had remembered the way off by heart.
