Close to the end of the preview for 'Folsom Prison Blues' there is a shot of Sam, Dean and a guard in a shower room. In the background it looks like Sam is either very interested in the shower controls or he is having a vision. I guess the guard could have punched him but I'm going with the vision idea.
Bad Timing
"Sam get moving the guard is coming!" Dean loudly whispered to his brother while ducking back into the bathroom. Not hearing a reply from Sam he walked into the shower room where Sam should have been getting the 'escape proof' grille off the entrance to the duct work. Instead he found Sam holding his head and grimacing in pain. "Ah geez Sam, bad time for a vision."
"Not…my…idea," Sam gasped out as the pain increased. "AAAAAH! Dean!" groaned Sam as the intensity of the pain caused him to hyperventilate and he started to black out. Dean grabbed him just as his knees gave out.
"Sammy." His concern grew when he heard the guard enter the bathroom. Dean half carried, half dragged his brother over to the shower wall. "Sammy you have to stay awake, the guard is coming," he whispered as he kneeled next to his brother. Sam barely nodded as he leaned against the wall but he managed to stay sitting up on his knees when Dean let him go. Before Dean could turn around the guard came into the shower room.
"Hey what's going on in here!" the guard pulled out his baton and reached for his radio.
Dean stood up and turned around holding his hands out, palms up, showing he had nothing in them. "Nothing Sir. My brother wasn't feeling well so we came in here." The guard observed the younger man's pale, sweaty face and fast breathing and concluded that something else was going on.
The guard pulled out his handcuffs and pointed at Dean with his baton, "okay buddy, get over there, face the wall and put your hands on the back of your head." Dean glanced back at Sam before turning quickly and hitting the guard with a right hook. The guard staggered back but managed to stay on his feet and swung at Dean with his baton. Dean blocked the baton but he wasn't able to block the guard's left hook and stumbled back. The baton was swung again and it would have connected with Dean's head if Sam hadn't hit the guard with a flying tackle. They landed hard on the tile floor and rolled against one of the shower stations. Sam dizzily laid against the station while the guard got to his knees and drew back his arm to punch him. Before the guard could throw the punch Dean kneed him in the face, breaking the man's nose and knocking him out.
"You okay Sam?" Dean said as he helped his brother to stand up. "You aren't going to hurl are you?"
Sam wavered a bit, "yeah and no." He glanced down at the guard on the floor. "We need to get the hell out of here Dean." He held onto Dean's shoulder as they walked over to where the 'escape proof' but not 'Winchester proof' grille laid on the floor.
"Okay Sammy, you go first." Dean hunched over, ready to give Sam a leg up.
"No Dean, you go first." Sam held up his hand as Dean started to protest. "Dean I'm taller than you. I can jump and pull myself up."
"Fine." Dean put his foot in Sam's hands, "bitch."
Sam lifted Dean up to the duct opening, "jerk." He gave Dean's foot a shove when he felt Dean haul himself into the opening. Sam glanced around the room one final time before jumping up and grabbing hold of the edge of the duct opening. He grunted as he pulled himself up and into the duct. If their luck held they would be out of the Detention Center within 15 minutes, if not, Sam didn't even want to think about what the repercussions would be.
