It's set immediately after the current shinobi war, with Sasuke return back to Konoha.

She could tell his conscious was teetering over the edge — the side effects of the antibiotics had began to take its toll on the resilient Uchiha's body. The low rumble of his breath moved slower, and slower. The lines of his face grew softer, as his muscles uncoiled in the comfort of a stiff hospital bed. Sakura knew they weren't the best, but they're better than laying on the bloodied wreck of the war-zone grounds.
Sakura sat in the chair at his bedside, absorbed in the thoughts regarding her former —renewed?— teammate. He changed sides, fought alongside the teammates he once tried to kill. He claimed he wanted to become Hokage and protect the village. It was a complete 180° from his cry of wishing to destroy Konoha. Sakura couldn't understand what would make such a revolutionary change in the avenger's thoughts.
Suddenly, sitting there and watching Sasuke's beginnings of falling sleep wasn't enough. Sakura needed answers to the turbulent questions filling her mind. So even though she knew it was her job as the man's medic to let him sleep, she couldn't help but take advantage of his position and ask the one question she could, "Why are you back, Sasuke-kun?"
Even in the dark of his hospital room, his infamous eyes still stood out. The color was unlike any of the surrounding shadows he could easily meld into — and was something Sakura could not look away from. The insides of her stomach twisted and the bubbling of anticipation clawed its way down her spine as she waited for his answer.
"Sakura," he replied, voice deep and thick with the slumber slowly encompassing his conscious. She waited as patiently as she could, desperate to hear his explanation. It was almost too good to be true that he had returned, after all the years Naruto and her had worked so diligently to being him back.
"Becoming Hokage, and rebuilding my chan's status in Konoha, with you at my side…" Sasuke veered off track, suddenly too tired to say much else. Sakura held her breath, silently begging him to continue. His voice was becoming softer and softer, indicating that the drug was finally shutting his mind down, "..I would look forward to a future like that."
