Jerry's gun went off and shot him in the butthole, traveled up and pierced his skull.
*close up on Jerry bleeding from the head*

Unfortunately, there was ricochet and McGarrett got popped in the temple and died.
Danno walks into the scene:
"NOOOO! McGarrett! I cannot live without you!"
*grabs gun, pops one into his own temple*
…AARRGH!
*Danno dramatically and tragically croaks*
Ricochet like in a pinball machine killed Kono, Kamekona, Chin and that broad y'all hate, Catherine.
Wo Fat, who wasn't really dead, it was his clone all along, turns Hawaii into his personal crime mecca.
