It could be because Sam hasn't had his soul for how many months now? Or maybe he's just not used to seeing Dean being this adorably pathetic when he's sick. Spoilers for the second half of Season 6.

"M' not sick", Dean mumbles before coughing and sneezing incessantly, moaning in complaint when Sam slips the thermometer past his lips.
Yep, figures.
"Not even remotely sick huh, Dean?"
Dean looks beyond adorable lying in bed, covers tucked up to his chin, little nose stuffy and Rudolph red.
Should Sam cry or laugh at his brother's vulnerability? He's just glad his soul's intact so they're sharing this rare moment together.
Dean stares up at him, sweat dripping from every pore, and growls.
Sam laughs, brushes Dean's hair back while watching him reach for the remote, "Just go to sleep, bro."
