Dean and Sam loved each other but they weren't the type to actually say the words. Sam was more than willing to but he knew Dean wasn't comfortable hearing it. They had been raised by John Winchester who had drilled the importance of being manly and tough into their heads. Emotions were weaknesses they couldn't afford, according to him. Sam thought that was bull but Dean had
always taken everything his father told him to heart. Anytime they got too close to an "emotional scene" Dean would get uncomfortable and crack a joke. So Sam would say I love you the only way Dean would let him.

"Jerk."

And Dean would always say it back the only way he could.

"Bitch."

The first time Dean had called Sam a bitch was when Sam was six years old and throwing a fit because he wanted to watch TV. Dean refused to let him till he finished his dinner. Sam had said,
"You're a jerk, Dean."

Dean came back with, "Just eat your dinner, bitch."

Sam's mouth dropped open and he said, "You said a bad word! I'm gonna tell Dad."

Dean knew dad wouldn't care though. In fact instead of Dean being punished, Sam would probably get a lecture about how he should follow his brother's orders when Sam told their dad the story. Dad had never even told them not to say bad words. It was Dean who told Sam about bad words.

When Sam was three their dad had taken them to a friend's house, a fellow hunter whose wife agreed to watch them while their dad went out. He'd been going on about how he had to find this fucking demon that was killing locals. When their dad told them goodbye that night Sam asked him if he was gonna go kill that fucking demon. Dean told Sam not to say that word. Their dad just told Sam to listen to his brother and left. Sam asked Dean if demon was a bad word. Dean told him the word he'd said before demon was. Sam asked why it was a bad word. Dean just told him that there are words that adults can say but kids aren't supposed to. Sam asked what the other bad words were. Dean named off the words he remembered hearing his mom tell their dad not to say around him. Sam promised he'd never use another bad word.

By the time Sam was ten Dean was using bad words pretty regularly. Sam still didn't use them but he called Dean a jerk on a regular basis. Dean would always call him a bitch back and Sam would just roll his eyes.

The first time they exchanged the names as an endearment rather than an insult was when Sam was thirteen and they had a hunt go bad leaving Dean with a particularly nasty gash across his stomach. The whole time their dad stitched Dean up Sam had paced back and forth, his arms encircling his body protectively and crying. When Dad had declared Dean was okay and left, Sam ran over and threw his arms around Dean. He had started babbling about how scared he was that Dean was gonna die and how glad he was that Dean was okay. Dean had told him not to be such a girl about it. Sam had looked up at Dean with tears in his eyes but a smile tugging at his lips and called him a jerk.

Dean grinned and said, "Bitch".
