She's a legend, not just in Seireitei but in all of Soul Society and probably most of Hueco Mundo too. Unohana Retsu-taicho has, apart from Yamamoto-soutaicho, the most far-reaching and, frankly, fearsome reputation of any Shinigami in Soul Society, including Zaraki Kenpachi.
Unohana-taicho has a reputation for a mostly calm, even, composed demeanor, but also for a temper that, on the rare occasion that it is aroused, is ferocious and earth-shattering, with the force to cut down all those who cross her path. Moreover, she is reputed to be highly protective of the Shinigami under her care and perhaps a more effective and responsible captain than most of the captains of the Gotei Thirteen.
Maybe that's why so many Shinigami flock to Unohana and the Fourth Division is the largest division of the Gotei Thirteen, despite the abuse piled upon it by the Eleventh and others.
Because the Shinigami of the Fourth know their captain will protect them from the worst abuse.
Hand in hand with her protective tendencies, Unohana-taicho tends to take a dim view of any Shinigami treating the members of her division with anything less than respect. When she witnesses it, the bullying is usually put to quits. Quickly.
Truth be told, that's the only reason any one would want to be in the Fourth Division, even if they did have a passion for healing and cleanliness.
Because Unohana's in it, that famous Shinigami, the living legend, and even if the Shinigami of the Fourth are scorned, they're also sure that she's on the look out for any trouble that might befall them.
