Summary: Mirajane Strauss was admitted to the Fairy Tail Hospital Wing on January Fourteenth 2014, this is the report that was filed.
Master Makarov sat in his office surrounded by piles of paperwork. Some from the Magic Council, some were requests to be filed and some were incident reports. It was a slow and arduous task, but it had to be done. At last he reached the final piece of paper in his 'In' Tray and read it slowly. With each word his eyes grew wider and wider and at the closing sentence, he keeled over in his chair and did not move for several hours.

Mirajane "Mira" Strauss.
What was the reason the reason for the subject's admission to hospital?
Due to a cardio vascular disorder commonly known as, "Overload of Fangirling". The cause of the Cardio arrest has been recorded by Levy McGarden and Lucy Heartfilla.
Who was involved in the incident?
Natsu Dragneel and Gray Fullbuster.
Were there any casualties?
No.
What was the cause of the incident? Please describe with as much detail as you can recall.
I was sitting on the bench, drinking my smoothie, talking to Levy about my book which was recently published when Natsu and Gray started yet another fight in the hall.
Mira was serving drinks when Natsu punched Gray in the stomach and stole his briefs. They then proceeded to call each other names.
"Dragon-Breath."
"Stripper."
"Slanty eyes."
"Droopy eyes."
"Pink haired freak."
"Snowflake."
"Hot-head."
"Ice-Princess."
"Mr. Fullbuster."
The final line was said with a smirk and air of finality.
At which point the whole guild fell silent, Erza's cake dropped off her fork, Cana's barrel fell to the floor with a clunk. Several people choked on food or beverages. Juvia dissolved into tears and I shattered the glass in my hand, blood and smoothie staining the Levy's copy of my book. Mira fainted, the tray in her hand clattering to the floor. Natsu stared with a confused look on his face but did not say anything more. After thirty seconds with no other insults from Natsu and the whole Guild burst into confused chaos. People regained their senses and recovered from the shock, but Natsu's expression still hadn't shifted. Gray took his underwear from Natsu and instead of putting them on he slung them over his shoulder and kissed Natsu on the cheek. Mira, who had just stood up again, squealed and dropped to the ground again.
Gray walked out of the Guild, in all his nakedness and Natsu, finally awake again shouted after him,
"You promised it would be Fullbuster-Dragneel! You lier! And put on some clothes! I'm the only one who gets to see you naked from now on!"
So, um... Heh. I don't even know.
