It wasn't a dream.

Ferb Fletcher stood, shaking, in a basement he'd never seen before, and knew in his heart that the horrifying phantom reaching for him wasn't a figment of his imagination. Because in a nightmare, his brother would be gone, and, right now, Phineas was clutching him in terror, screaming at his side. At least now they could die together. That was so much more than any nightmare had ever offered.

Ferb crushed Phineas against him – "Don't leave," he thought. "Don't die before I do."

But Phineas tore himself away.

"It's all in our heads," his eyes said.

It was supposed to be a bit confusing. I consider author's notes explaining what happened in the story to be cheating (I mean, if the story is unclear, then it needs work), but this was supposed to have a bit of a surreal feel to it.

If you don't like the surreality, then I'll spoil it for you and tell you that what Phineas was saying was that the FEAR was all in their heads - there was no need to be afraid 'cause the phantom was just a robot.
