As Sam was screaming in agony in Bobby's panic room, the effects of the demon blood slowly and painfully fading away, he knew something was different.
The blood, what it made him do, it changed him.
He felt tougher, but not stronger.
He felt broken, but not hurt.
He felt faithless.
He feared the moment Dean would come visit him, even though he was everything Sam needed.
He betrayed his brother in the worst way possible. He was a monster.
But when he looked into the concerned eyes of his big brother, he knew that some things would never change.
