Sam Winchester was a lot of things, but stupid wasn't among them. Naive, yasureyabetcha, he, when being honest with himself would admit to.
He KNEW deep in his soul something was different since waking up in the Impala. Sleeping, sure he could see that, but the sudden return of some of his health was a little more tricky. One simply did not stop coughing up blood for no reason, nor begin moving freely after months of severe aches and extreme pain.
So, something had happened, Question was what deal had his idiot, loving, older brother made this time? Cause contrary to what he told Dean, he did remember talking to Death and wanting to finally let go of a lifetime of regret and sorrow.
Sam did understand where Dean was coming from. Not just the whole protect him thing, but Dean's great fear of failing family and being alone. A fear he was intimately familiar with inside himself.
One of the memories he hadn't shared was seeing a bright light, a face he didn't recognize and a feeling of being tugged away.
Sometimes he could feel a presence inside. A presence he associated with that face. If he was very careful, he almost felt as if he could catch whatever it was. It felt benign, but being Sam Winchester made him more than above average paranoid.
It was hard to chase and so far uncatchable. He knew deep in his gut that this was not an illusion, but something real. Question was, what was it, and how did it relate to whatever Dean had done to save him? If asked, he couldn't explain it, but he knew they were related.
Eventually he would either "catch" whatever was inside him, or Dean would break and confess. Until one of them happened, he would be calm, as patient as he could be, and keep healing.
Stay on his guard, sure, keep trying to figure out what was hiding inside him, watch Dean carefully and wait.
Waiting was fine,
