This is just what I think Carmen saw before she told the Doctor the prophecy about his regeneration. Obviously some spoilers for "Planet of the Dead"
THE PROPHECY
Carmen breathed a sigh of relief. She was so relieved that she and Lou had survived the Swarm. And it was all thanks to the Doctor and Christina.
She smiled, thinking of them. Those two were a wonderful team, she thought. They had managed to keep everyone calm and busy enough to keep their minds on the job. If they hadn't been able to and had just lost their heads in panic and ran into the portal like the bus driver had … Carmen shuddered. She didn't want to even think about the implications of what may have happened.
There was also a feeling of pride going through her. She had managed to help a lot with her … "ability"; she supposed she would call it. The same ability that helped her and Lou win ten pounds twice a week from the lotto draw. "It's amazing", she murmured to Lou, "how something that we use for something so small helped save the Doctor and Christina from the Swarm." Lou nodded in agreement.
She turned around to say goodbye to the Doctor, when she suddenly had a terrible vision. She saw the Doctor, writhing in pain! And darkness. Darkness everywhere. It was like a completely black cloud had fallen in front of her. And when she tried to look through the darkness, she saw something coming through it. It was too small to tell what it was, but it was coming at her fast! She didn't know why, but she had a vague feeling that it had been gone for a long time and was returning. And then, she heard a sound, a sound of someone knocking four times. She didn't know why, but she just knew it was a man.
Just as fast as it had come, the vision ended. Carmen was shaken badly from her vision. After she recovered slightly, she knew that she had to tell the Doctor about what she saw. Somehow, she knew it was vital that she tell him.
As she was walking towards him, she couldn't help but think that the Doctor was going to die, that his song would finally end. With that thought, Carmen silently prayed that he would survive.
The universe needed the Doctor.
