Ganjyu did not see Hanatarou as a girl. He simply did not. Hanatarou had a penis, simple as that. Unohana is not completely surprised to see Yamada come in late the next day and with a noticeable limp, a contented smile on his face. In the end Ganjyu had been convinced to wear a condom after all, truth be told, Hanatarou spent the whole night proving to Ganjyu-san that it felt the same with and without a condom.
