Itachi knew he was going to die at some point in his life. Not by old age or in peace.
Especially after the massacre he commited, karma wasn't the reason either.
Even after he discovered his sickness, he knew that that wouldn't be his death, even if it was damaging, destroying and tearing his body from the inside apart. It was just a side effect of death.
Being a shinobi meant that death was lurking around the corner, you'd never know when your time came. A shinobi that lived out his life was a rare one.
Akatsuki was an one of a kind group, that wasn't afraid of dying. They didn't fear death only embraced it, played with it, made fun of it and honored it. (Especially Hidan)
Itachi knew when his time would come, he knew when it was time to die for him. Not because of sickness, karma or old age.
He wanted to be killed, to pay for his sins, even if they were made out of love. Love for the village, his family, friends and his little brother.
Although he knew he wasn't going to die in peace, he felt peaceful. He was fianlly free of his sins. Freed by his little brother. He knew he deserved to die, he lived long enough to know what the world was made of.
He knew his end was coming, it was coming to get him. Finally free from the curse of his sins. He knew this was not the end.
That was one thing he knew...
