Dr. Tenma is in the operating room ready to treat his patient. As he looked at the description, the patient's name is Kazuhiko Amamiya. However, he knows that this isn't the real patient's name. It's Yosuke Kobayashi. As Tenma recalls on his mind about him, he has watched the news that he was a police detective trying to look for a serial killer who mutilates his victims by severing their limbs. After the horrifying night with what happened, Yosuke gets transferred to prison as he was accused of murder. However, something bad happened to him. His name is changed into Kazuhiko Amamiya. It seems that he has multiple personality disorder, a condition where two or more personalities existed in a person. Personalities will change depend on the environment. As Tenma finished looking at the description, he looked at his patient.

"Now I'm ready to save his life," he said, looking at what happened to Kazuhiko. He found a bullet wound at his head. Of course, Tenma's specialty in doctorate is neurosurgery. Since the man he will operate has multiple personalities, he puts a brain scan on him. Looking at the X-rays, Tenma found that within Yosuke, his original personality is gone, replaced by Kazuhiko. Wait, there's a second personality within his head and that is Shinji Nishizono. Tenma realized that not only he's going to save the cool-headed detective, but also a psychopath. He gets shocked by this, wondering if he should do the operation. If he let his patient die, it will not only be the end of Shinji, but also Kazuhiko.

"Is everything okay, doctor?" asked the nurse. "We should get started."

Tenma took a deep breath. He is ready for the reply.

"Okay, let's get started. Let's get the bullet out of his head."

He successfully removed the bullet from Amamiya's head. This act not only saved him, but also his alternate personality, Shinji. As he looked at his life rate, it looks fine. Looks like he'll be living in this after all.

I can do it. I can save both personalities after all. Tenma thought.
