Dean hates that he has to make this choice. He hates himself for always ending up here. Because he knows-before he even thinks it in so many words-what he has to do. There was never any contest. And that's not fair to Cas, but nothing has ever been fair to Cas.
Still, when he sees the look in his friend's eyes, Dean wants to come clean. He wants to tell Sam everything in hopes that it will somehow solve this. But the thought of Sam dying keeps Dean from letting the truth slip.
But he has to say something.
"If there was any other option..."
And Cas seems to understand then. He gets this knowing look in his eyes, and nods once.
"Okay," he says.
It's a heartbreaking sound, but Dean has to believe he means it because he can't think of Cas getting hurt because of him.
Sam is the only choice. Always has been, always will be. After everything Cas has done for both of them, Dean knows it isn't right. He knows Cas deserves better. He can only hope for a chance to make it right in time.
They always make things right in the end.
