A/n: Missing scene from "Sympathy for the Devil", the first episode of season 5. Very mild limp Sam.
Contains spoilers!
Takes place just after Zachariah attacks Sam and Dean.

It is over in an instant. Zachariah is gone and the room is eerily silent, save for the ragged gasps of breath that shatter the silence and abruptly bring Dean back to reality.
Breathing…
Sam.
Dean is on his feet in an instant. He runs a hand over his own abdomen.
He is completely healed.
His eyes fly to Sam, who is still hunched over on the floor.
Sam is eerily silent.
Dean's eyes widen with concern and he hesitatingly leans towards his brother, unsure of Sam's present condition.
Sam suddenly draws in a sharp breath, and the pause before his next breath is interminable.
"Sam!" Dean says sharply. His voice is gruff, but the edges of his tone betray his carefully-guarded worry for his brother.
Sam's head rolls back and he looks up at Dean with wild, fearful eyes. He clutches at his chest and takes hungry, painful breaths as he struggles to stand.
Dean steps forward haltingly. He holds back his desire to offer a hand or a shoulder for support.
The chasm between Sam and Dean is bridged by his brotherly instincts for an instant.
Dean gives in.
He reaches out a hand, and it hovers outstretched before Sam.
Dean is shocked when he feels Sam's hand in his.
Sam's hazel eyes are glassy and confused. He fights back the drowning sensation and stands awkwardly to his feet, swaying forward into Dean's awaiting arms.
"Sammy," Dean says. His voice is firm and quiet. He no longer says Sam's name with the old tenderness.
"Can… can you stand?" Dean asks after a long, tense pause.
"Yeah," Sam answers hoarsely, brushing Dean's arm away.
"Ok, ok," Dean says, stepping away quickly and brushing himself off.
Castiel regards the brothers with a solemn gaze. Another angel might have recognized curiosity and even surprise in Castiel's ice blue stare.
Sam takes a deep breath and realizes his newfound appreciation for the subconscious respiratory functions of his lower brain. He blinks several times and looks at Dean with mild surprise and carefully guarded concern.
He can tell that Dean is fully healed.
Sam rubs the back of his neck absentmindedly as he shifts on his heels and looks at Castiel.
The angel avoids Sam's stare and locks eyes with Dean.
He can see relief in the older Winchester's stony glare. Castiel knows that Dean can sense the impending destruction and horror that awaits them, but can sense that Dean has found momentary peace.
Zachariah is gone and Sam can breathe again.
Castiel also notices that Dean is still standing behind Sam, ready to catch him if were to fall.
