This is just a small idea of mine. If given enough support I'll make it an actual series though.

He doesn't remember much of the time before he was introduced to the sea. Sometimes all he remembers is just sadness. He must have been a brooding toddler. What he does remember is the conversation though, and its what led him to this. He's pretty sure that this is his death and God's above did he have little regrets.

They are there, no one, no matter how much they look forward and don't look back, have regrets. He regrets not visiting Winterfell more often. He regrets not being there for bran, though he knew little about him, he was his brother wasn't he?

And then there was his regrets on the sea, well - there was that time when... Or maybe!- no, no that's not it. Does he not have regrets in the sea that will secure his death? He chuckles, so this is the end of the infamous Jon Snow. Or is it targaryen? Maybe stark? They don't quite have the same ring.

"Ghost..." his hoarse voice murmured. He left him at Winterfell, will he remember their time together after a couple of years? Will he even live on as the legend he hoped people would remember him? Maybe his siblings will live on to tell his story?

Arya and Bran will most likely carry it on, Sansa may not even want to talk about him,and Robb?

Robb...

He hopes Robb is the one whom continues his story. he finally draws what he believes is his final breath. Death seemed easier than falling asleep.

after what felt like an eternity, he wakes up gasping for breath in a room. Hes in a comfortable bed. His first thought before any other? 'How am I alive?'
