I imagine Kreacher has changed quite a bit since Narcissa saw him, thus the disgust. Though, of course, she always found house elves disgusting. Narcissa wanted to crumple her nose and pull her robes as close to her as possible to stop any risk that the creature would get dirt on her. Every part of her urged her to do that, but, instead, she knelt as gracefully as she could in front of him.
"Kreacher," she said softly, attempting to force compassion into her usual ice cool voice, "Can you help me? Please, I would very much appreciate it."
The house elf nodded enthusiastically and Narcissa allowed herself to smile. If this worked, the Dark Lord would forgive Lucius and all would be well.
