Author's Note: This is what J.D. realizes at the end of the episode My First Day. I tried to stick closely to the episode, but I might have strayed off and it is all from J.D.'s point of view.
J.D. Narration :" See every story needs a good guy. If Kelso is the jerk then who is the good guy?"
After this thought I was paged and I went running in the room. There I saw Carla and Dr. Cox rushing in a car accident victim. They were working hard on him and for a minute there I didn't think they noticed I was in the room with them, until Dr. Cox turned to me and told me to relieve the pressure from his chest. As soon as he said that I got nervous.
Then I started to freeze up. I didn't want to make the incision into this man and then get a chest tube and push it into him. It is a funny thing, that my ability to do this act could in the end save or kill me patient. Unfortunately, I got called back to the real world by the sound of Dr. Cox telling me
" We have got to relive the pressure in his chest. J.D., you do it."
" No , No, OH! God! No!" I screamed to myself.
Dr. Cox looked at me and for a minute there I thought he was going to take over at that very moment and send me home packing. He did not do that instead he said this
" Look at me. You can do this."
The strangest thing happened to me. I believed him so without another minute hesitation I made the incision into the man's chest. Then I asked for the chest tube and I was trying to get it in the cut I made, but I had trouble getting passed the flora. So, I told Dr. Cox just that and he told me this. " Well don't be gentle, get it in there."
I finally did it and we found that he had a normal rhythm. I was about ready to step back and let Dr. Cox take over, but Dr. Cox just took off his gloves and then he looked at me and said " He is your patient now, doctor." then he left the room.
In that moment I realized that Dr. Cox scared most of everyone in the hospital but deep down everyone figured out that he was the good guy in our story. He actually cares for people even though my first impression was to run screaming like a little girl away from him because he is so scary on the outside, but when it matters he the one who makes me believe him and teaches me the most about being a doctor. Dr. Kelso might be the chief of medicine, but he is certainly no mentor, Dr. Cox however is.
