Look into my heart. You'll never find what you're looking for, unless I want or hope that you'll find it.
My "friends" have searched, but I think, that only one has found it.
My heart is black, like my mind. You'll need my light to search for my heart
Maybe I'll give it to you. But the chances are small.
Will you be the one to find it what's everyone is looking for?

So this is a poem that Sasuke is saying to who ever you want him to say it to. But basically it's to show how only one person have found his heart and managed to become his friend.
