Sam was hiding things about himself and his nightmares, supernatural things, possibly evil things.
Dean was trying desperately not to let his unease show as they pulled into the filling station. Sam claiming to have visions about the future, so here they were on their way to their old home.
Dean didn't want to face his memories of that night. Much less face them with only Sam as backup.
Lying to Sam about going to the bathroom, Dean rounded the corner and pulled out his phone.
"This is John Winchester, if this is an emergency call my son Dean at (866)907-3235"

You can't call yourself when you're the one that needs the help.
This scene was based on one of the first true deceptions and keeping of secrets between the brothers. Sam finally admits to the truth about his nightmares and blaming himself for not saving Jessica, facing his own guilt in ignoring his visions. He tells Dean the truth to try and save someone else in spite of knowing Dean's mistrust of anything supernatural. Dean calls John behind Sam's back desperate for help and advice because trusting anything supernatural even Sam is anathema. Dean's trust in his brother is starting to be shaken because up until now almost everything supernatural was inhuman and evil. Kill it and move on, has always worked before. Now not only is it human, it is Sam the little brother he has always tried to protect, but how do you protect someone from themselves?
