Her spiritual daughter
When Sakura had announced her pregnancy, Tsunade suddenly felt the weight of the years slamming on her shoulders. Sakura, her student, the little genin she had trained, was married and soon to be a mother. She had looked at her, wondering where the teenager she had agreed to take care of went.
One second, she was Sakura, thirteen, physically weak but strong willed, with a lot of potential.
One blink of an eye and now, she was Sakura Uchiha, a strong kunoichi, one of the best medic-nin in the village, proud and rightfully so. She had mastered all of her teachings, she had married her first love and now, the fruit of their union was blossoming in her womb.
To be honest, Tsunade had always liked Sakura.
She had reminded her so much of herself! She had liked her bravery, how persistant she was. So many would have quit! Sakura persisted. To see her evolve, progress, her friendly banters with Ino, it made her feel like a mother. Yes, for some reason, Sakura was almost like a daughter to her. She was her heiress already. So the feeling wasn't all that strange. She had always felt that wave of pride everytime Sakura reached a new step in her ninja training.
Tsunade never had the chance to have a daughter of her own, nor did she have the chance to birth a child, but had she been able to have one, she would have wanted that baby to be just like Sakura. A hardworking, honest, strong willed, brave child. Of course, Sakura wasn't out of drawbacks. But her qualities made those drawbacks little when compared to them.
She stood by her thought, in another life, if she had the chance to be a mother, she wanted a child like Sakura.
And while Sakura reminded her so much of herself, she was glad she seemed to have been spared from the curse that had plagued her life:
Unlike her, Sakura was having it all, something she deserved.
She was glad that, unlike her, she had some kind of fairytale happy ending.
