He knows she's sleeping with both of them. Quite frankly though, he doesn't care.
But he also knows that Rodney doesn't know. If Rodney did then he would break it off and John can't have that.
Because if kissing Elizabeth Weir. If sleeping with Elizabeth Weir is the closest he's ever going to come to Rodney McKay… Well, John will take whatever he can get.
And sometimes, when he kisses her… If he squeezes his eyes shut tightly enough.
