During his time training with the Fourth division in healing kido, the thing Kira is most likely to be admonished about is his lack of focus, his inability to keep to the task. Kira was always—and remains—good at moulding reiatsu, but his focus on healing kido—which is strangely more difficult and complex than other forms of kido—is not always proficient.
Unohana gently admonishes him, some of his tutors less gently. In the end, Kira learns as much healing kido as he can, and when the time for the exchange program is up and Kira's sent back to the Third division, he takes his newfound skills at healing with him, however incomplete and substandard they may be.
Kira still lacks focus.
"Sorry, Matsumoto-san." His voice is bitterly quiet as he still tries to press healing kido down to the absent area of her torso where flesh should have been. Rangiku is still struggling for breath, and he can't understand how she can still be breathing with half of her ribcage and everything in it ripped out. "I'm really not up to snuff."
Rangiku probably can't hear him over the sounds of her own horrific, gory gasping, blood bubbling out of her mouth, each spurt with more volume than the last. Kira's stomach twists itself into knots, and he tries to push down the burning panic that she might die under his insufficient hands.
"Just hold on a little longer, Matsumoto-san," Kira mutters, brow furrowing as the sounds of the chaos and slaughter above him bear down on his hearing. The sound of her labored breathing is nearly unbearable. "This'll help until Unohana-taicho or someone gets here who can heal you better than I can. It won't be long now; I'm sure of—"
"Kira." Rangiku's voice cuts through his panicked blabbering.
With difficulty, she has lifted the bruised lids of her ice blue eyes. She's glaring, with as much heat as she can muster in her near-dying state. "…Shut up…" her voice is perhaps more harsh than it would be in other circumstances "…and hurry."
Reluctantly, Kira nods.
Focus, he can still hear Unohana-taicho telling him. Focus, if you want this patient to live.
It's not good enough.
But it will have to do.
