Sakura agrees not to touch Sasuke if Sasuke agrees not to kill Naruto and Naruto agrees to stay away from both of them.
It's hard at first, but when they truly become nin it turns into second nature. Carefully sidestep that wandering hand, wave away birthday hugs, opt for games with your reflection in the water, training silently in the middle of the night when no one else is around.
There are times when Kakashi-sensei notices their unwillingness to touch each other, and he attributes it to childish things like cooties and flirting and not so childish things like insecurity and fear.
They themselves really dont know when they started it, or why all they know is that things seem a little safer when they aren't touching.
In some ways, they became the perfect ninja.

So, when Sasuke has gone and become a traitor, Sakura leaves and makes a living as a famous medic in the sand, and Naruto becomes hokage, they all respectfully stay far away from each other.
Sakura doesn't chase after Sasuke, and in return the raven keeps his distance from the village of leaf. Naruto, in accordance, never asks Sakura to return, doesn't spare her a glance when he passes her by in the crowded streets of sand on the way to a meeting with the kazekage.
It hurts a bit, but it's okay, because team seven has an...
