Orochimaru catches him before he makes it to the border. Kabuto stands off to the side; head tilted as he watches another of Sasuke's dreams die. Sasuke follows them three years later; unseeing, unfeeling and rotted from the inside out.

And Sasuke makes it to Konohagakure
Jiraiya finds him deep in the forest though he, himself is unsure how since he wasn't looking for him. Maybe Naruto is right. Maybe Sasuke's not Orochimaru and maybe the story will change. Jiraiya asks a question and Sasuke answers wrong. He slits his throat and washes the blood off his hands as he waits for the anbu to come and bury the body.

And Sasuke makes it into Konohagakure.
Tsunade comes to him when she can. There is no conversation since she says nothing, and his tongue was burned out long ago. She lifts a brightly gleaming scalpel and Sasuke thinks that if he could, he might laugh to think that some time long, long ago, of the legendary sannin, he was least afraid of Tsunade.
He has since learned better. After all, the best medic nins, the legendary, can keep a person alive for a very long time.
