Chapter 19
Monday
Miaka woke up five minutes before her alarm went off. It was set at 5:45am. Stretching she got up and prepared her coffee maker before heading to the shower. It was her first day on day shift. While in the shower. She noticed how light she felt after letting go of the heavy burden she carried for three and a half years. She felt good. Quickly finishing her shower she dried off quickly and picked out her scrubs for that day. Felicity had messaged her that the color they were wearing was orange scrubs and that the guys were wearing black scrubs. Picking the Koi Kathryn Lucky top and orange Lindsey scrub pants she checked herself in the mirror. What stared back at her was a woman who had gotten back the sparkle in her eyes and she knew it was because of a certain man, who was the reason she had taken the leap of faith.
Rafael..
The reincarnation of the gentle emperor who had given his undying promise as he breathed his last. Now as Rafael he had come back to fulfill his promise, even without his memories of being Hotohori. She smiled before heading over to the kitchen to pour herself a cup of coffee. Taking a sip of her coffee she toasted up some wheat bread and a bowl of cereal. Setting her cup on the counter she started eating her cereal and once the the toast was done, she placed some butter and strawberry jam on it and ate.
After she had finished her coffee, she went to the bathroom, brushed her teeth and applied a little bit of make up on her face. Once she was done, started to fix her hair, braiding it to have it fall on her right shoulder and her bangs fell on her forehead. She packed two of her orange plumeria foam flower clips into her purse.
Once she was done Miaka started preparing her purse, her pens and her ID. Once she was sure she had everything, Miaka then donned her protective gear. Since she was taking her Ducati to work as to not deal with too much traffic. Glancing at the clock, she saw it was 626am. Miaka prepared to leave when she heard her phone beep, indicating a text message. She paused and read the message
[Rafael]
Good morning my pretty lady. I hope you have a nice day at work.
Miaka smiled at his message and replied back.
[Miaka]
Good morning Rafael. Thank you for the message. That cheered me up. I hope you have a nice day at work too.
She got her keys out and locked her apartment. Heading to the garage. She placed her purse in the side bag of her Ducati and hopped on. Sticking the key into the ignition. She turned it on to warm up the engine. After a few minutes, she put on her helmet and took off for work.

Rafael had just parked his BMW Z4 at the Physician's parking at UCLA, when he messaged Miaka to greet her good morning. Dressed in a suit and tie. He headed over to the medical building where the residents were going to meet for lectures and such prior to heading to the hospital.
Putting his phone on silent mode, he entered the building and made his way to the conference room, where he found already two other residents talking in the room.
"Good morning. I'm Aleksei Romanov." One of the two residents introduced himself. Dr. Romanov was of medium height, muscular build with blond hair and green eyes.
"Good morning, I'm Angel Ruiz." The second resident introduced himself. Dr. Ruiz was tall and lean, with curly brown hair and light brown eyes.
"Pleasure to meet you both, Dr. Romanov. Dr. Ruiz." Rafael replied shaking both proffered hands "I am Rafael Shu" he introduced himself.
As he sat down to join in the conversation. The door opened to reveal three more people. Two women and a man walked in the room.
"Hello, I'm Sota Apostolakis." One of the women spoke up. She was medium height, slim with shoulder length Black hair and green eyes.
"Hi, I'm Joanne Miller" The other woman introduce herself. She was also medium height with a more athletic build, strawberry blond hair cut into a bob and blue eyes.
"Good morning, I'm William Holmes." The man introduced himself. He was tall and slender, with platinum blond hair and blue eyes.
The three other men introduced themselves and shook their hands.
They all then sat down and talked while waiting for Dr. Bonner. While they were talking Rafael felt his phone vibrate. Checking it he saw it was a message from Miaka.
[Miaka]
Starting my shift soon. Have a nice day.
He immediately typed in his reply.
[Rafael]
Don't stress yourself ok. Say hi to the rest of the group from me.
After sending it he placed his phone back into his pocket. That's when he noticed that Dr. Miller was looking at him.
"I guess that would be your girlfriend" Dr. Miller stated.
Nodding his head, Rafael remained silent. Only a small smile on his lips was present.
"I hope your girlfriend understands how busy you will be." Dr. Ruiz said
"I hope so too. But then again she works in the healthcare too" Rafael answered.
"That makes it a little easier."Dr. Apostolakis said. "My husband almost went crazy, but he survived." Laughing as she said this causing everyone in the room to laugh as well.
The door opened to reveal Dr. Mac Bonner. He walked to the middle of the room and spoke "good morning, I am Dr. Mac Bonner..."
The room fell silent as they listened to the distinguished Doctor speak about what was going to happen on the course of their fellowship.

Miaka, Yui, Ai, Felicity, Drew and AJ were in the nurses station with six other nurses waiting for night shift to give their report for change of shift. Miaka had put on her plumeria foam flower clip and was sipping on some Starbucks Tazo Green tea that Drew had brought in for everyone. A few minutes later, the night shift began to filter in the station.
"So you start days today" Adrian said as he sat down to give Miaka report.
"Yeah, but I think you can handle night shift without me." Miaka laughed. "They need me to counter against Jordan" she whispered
"Uh huh, you're right. but we can't compete against the A-team" Adrian replied referring to Miaka and her friends. "Ok let's start, all your patients are from Dr. Bonner."He added.
Taking out her pen and the hand off report sheet, Miaka took report for her patient load of the day.
"So today you have Rachel Fallows, 56 years old, full code, she's status post cardiac cath with history of multiple stents, hypertension and history of chronic A-fib. She's allergic to sulfa, codeine and penicillin. Cardiac rhythm is A-fib with heart rate ranging from 80s to 110s..." Adrian went on dictating about the patient history, and important details while Miaka wrote down everything she needed for notes.
"Next patient is Arthur Gambler. You have to be careful with this one." Adrian warned before continuing with report.
Looking up, Miaka asked "why is that?"
"Several code greens have been called on him and he already manage to injure Marie, the technical help." The other nurse explained as he continued to give report. "He's status post CABG, ethanol withdrawal with history of ethanol abuse. Full code, NKDA, non-compliant, cardiac rhythm is sinus rhythm with bundle branch block..." The other nurse reported
As Miaka took report she began to formulate a plan on how to deal with the non-compliant patient. She interrupted the report by softly saying "remind me to throttle Jordan later."
"Ok your third 3rd patient will have a procedure done today. The consent has been signed by The physician and the patient. Pre-op paperwork has been filled up too. Dr. Bonner wants to do a synchronized electrical cardioversion today since he'll have a bunch of residents going for their fellowship." Adrian narrated.
"Ok wait up! Is That Barton Inklers?" Miaka asked.
"Yes. He's cardiac rhythm changed from sinus rhythm to a-fib several days ago so Doc wants to deal with it." Adrian replied as he continued to give report.
"Finally, your last patient is Anna Hall. She's a status post CABG pt from last night. The midline chest incision is still covered with dressing, but it's is dry. Allergic to aspirin. Cardiac rhythm is Sinus Rhythm. She has her heart pillow in order to help her brace when coughing" Adrian narrated the report for Miaka's last patient.
After report, the group gathered together to talk about their patient load. They gathered up their paperwork and MARs and waited for Jackie, one of the resource nurses to pass out their phones.
"I have all of Dr. Bonner's patients today" Miaka told her friends as they sat down checking their patients' history and physical, current labs and the previous shift's notes on the computer. "Plus I'm on the code team. He's going to do a bedside cardioversion on one of his patients 10am so I need to finish passing meds on the rest of the patients before I get busy. And then I have the code green patient."
"I have three of Dr. Bonner's patients and one of Dr. Aldee's patients." Yui said as she checked her patients "I'm also on the code team with you"
"I have Dr. Montiel's patients today and one of them is on hemodialysis right now." Felicity said.
"Same here. I have her patients too. I might have to give a unit of PRBC if ever the result of H&H on one of her patients is below 8 again." Ai replied.
Drew spoke up next "Well I have Dr. Zigler's patients and one of them has a magnesium level is 1.1 so I'm going to infuse four grams of magnesium and then one has a heparin drip that I have to titrate depending on the lab result later."
Lastly AJ told them his patient load "I have two of Dr. Floyd's patients and two of Dr. Bach's patients. Dr. Floyd's patient is on dialysis too so I'm free at this time. Miaka, Jordan shouldn't have given you that code green patient."
"That's right. They should have given him to Paulo, Drew or AJ since they're the only male RNs on the floor today." Yui commented.
"I tried to talk to Jordan earlier, but he said that he is usually calm during the day." Miaka said.
"I'll do his meds at 9am." AJ offered "so you can prep for the cardioversion"
"Thanks." Miaka smiled gratefully "I owe you"
"No prob" AJ replied.
"Ok guys let's get to work" Ai said as she hustled everyone out.

"So today we're going to be heading to one of the Telemetry floors. The seventh floor telemetry unit has 48 beds." Dr. Bonner explained.
"Excuse me Doctor" Dr. Romanov interrupted
"Yes, Romanov" Dr. Bonner addressed him.
"There are two telemetry floors in this hospital. So why are we going to the seventh floor?" Dr. Romanov asked
"The reason being is that I send all my patients to that floor. I hold the nurses there on high regard and they are very competent. It's a personal preference that I send my patients to that floor." Dr. Bonner answered.
"I see." Dr. Romanov said.
Walking towards the elevator, Rafael looked around the hospital. His half ponytailed hair hanging down his back. Wearing the white doctor's coat over his suit. He silently. Listened to Dr. Bonner and Dr. Romanov's conversation.
"So why are we starting at the seventh floor, Doctor." Rafael finally asked As they all got in the elevator it was 9:35am.
"Shu, I have a patient that I have to do a synchronized electrical Cardioversion at 10am and it would be good for all of you to be there anyway." Dr. Bonner answered.
"The elevator opened with a soft ding on the seventh floor. All the doctors walked out and followed Dr. Bonner.
As he walked out Rafael thoughts strayed to Miaka. He sighed inwardly and berated himself Miaka is at work. I'll probably see her tomorrow night. With that thought, He followed the rest of the Doctors to the Telemetry unit.

Miaka was in Anna Hall's room finishing her medication charting when her phone rang.
"Telemetry unit. Miaka speaking" she answered.
"Miaka, it's Gilly. Dr. Bonner is here already." Gilly, the unit clerk said.
"Ok, I'll be there. Thanks" Miaka said ending the call then turning to her patient saying "Ok Anna, just continue using your heart pillow as a brace when you cough. Also your incision site is really looking good so hopefully you'll be discharged in a couple of days.
"Thank you, dear" Anna said "I'm so lucky to have such great nurses.
"Thank you, Anna. I'll see you later."
Miaka said as she exited the room.
Walking briskly to the station she went in and saw Dr. Bonner standing in front of the telemetry monitors.
"Good morning, Dr. Bonner." Miaka greeted.
Turning at her voice, Dr. Bonner looked startled seeing her and said "Miaka, I didn't know you work day shift too?"
Laughing a little, Miaka explained "I don't work nights anymore, Doc. I started working days today."
"My my, so i am really sure my patients are really in good hands since all of you are on days." Dr. Bonner said referring to the rest of her friends. "Are they working today too?"
"Yes, Doctor." She answered "but the Twins, Adrian, Gina and Joy can handle the night shift."
"Good good. Gilly, can you please call all the nurses so I can introduce the residents taking their fellowships with me" Dr. Bonner said.
"Yes, Doc" the unit clerk answered. As she called all the nurses into the station.
The nurses filed in the station, leaving the floor to Jackie, Jordan, Tony and Luz. All the nurses were whispering and writing on their hand off sheets. Miaka went to the back to stand with Yui, Ai, Felicity, Drew and AJ.
"Good morning nurses. I would like to take this opportunity to introduce to you all the residents who are starting their fellowships with me. You will be encountering them for a while until
They complete their Fellowship." Dr. Bonner started. Turning towards the other door, he motioned for the doctors to come in.
As the doctors filtered in, Miaka's eyes widened when she recognize a certain tall, slender figure with waist length dark brown hair tied into a half ponytail.

Looking at her friends, they all gave her a look that meant are you serious!?"
Yui leaned over and whispered to her ear "you didn't know?"
Unable to speak, Miaka shook her head
"This is going to be very interesting" Drew said under his breath
"Tell me about it" Miaka answered in a quiet voice.
When the doctors turned to face the nurses, Miaka eyes met Rafael.
"Ladies and gentlemen, kindly introduce yourselves to the nurses." Dr. Bonner said.
"Hi I'm Dr. Sota Apostolakis." The dark haired doctor started the ball rolling.
"Hello everyone I'm Dr. Joanne Miller." The strawberry blond woman said.
"Hi I'm Dr. William Holmes" a blond haired man introduced himself.
"Hello I'm Dr. Aleksei Romanov" the other blond haired man spoke.
"Hello I'm Dr. Rafael Shu" Rafael introduced himself, his face was unreadable.
"Hi I'm Dr. Angel Ruiz" the curly haired doctor said.
Rafael was in a state of shock. He couldn't believe that he was doing his fellowship in the same hospital where Miaka was working! He was turning in his mind what were the odds of that happening. He schooled his features so they wouldn't reveal anything. He noted that Yui, Ai, Felicity, Drew and AJ were looking at him quizzically. He gave them a small nod. He noticed that Miaka was looking down, pretending to look at her notes. Rafael wanted to go to her. He knew she was probably wondering what the hell was happening and all he wanted to do was to just take her away and hold her close.
The introduction soon finished and everyone filtered out of the room. But before Miaka and her friends could leave, Dr. Bonner called them to come over.
"Doctors, I would like you to meet the A-team of this Telemetry floor." Dr. Bonner said a hint of pride in his voice.
"Doctor, that's a little too much praise." Drew said.
"Nonsense!" Dr. Bonner said "You all deserve the recognition. Why do you think I only send my patients here?
Ok introduce yourselves"
Felicity spoke up "I'm Felicity, and these are Miaka, Ai. Yui, Drew and AJ" she said as she pointed at each one.
Checking the clock, Miaka saw that it was exactly 10am. Taking a deep breath she spoke up "Dr. Bonner, as much as we all want to stand and talk over here. It's 10am"
"Ok then let's get back to work." Dr. Bonner said.
"See you later." The said in unison to Miaka who acknowledged them with a nod.
Turning around and grabbing her patient's chart from the rack, Miaka handed it to the doctor.
"Mr. Inklers has been on NPO for 12 hours, D5 1/2 normal saline has been running at the rate of 75ml/hr. The consent and pre-op paperworks have been signed and completed. The portable monitor and the 12 lead ECG machines are in the room. He's also on two liters oxygen via nasal cannula at this time. His potassium level is 4.3 and he has been on warfarin for over a month already. His INR are within normal levels." Miaka narrated, carefully avoiding Rafael's gaze at her. "The only thing is that we cannot sedate the patient here on the floor." She added
"Oh yes, that is true. But you can go to the ICU overflow for procedure since you have your CCRN and CRNA certifications? Dr. Bonner asked her.
"Actually Doctor, everyone in the group has both certifications" Miaka answered keeping her voice level and carefully avoiding Rafael's eyes that were pretty much trained on her. "But I think the manager and the lead still need to approve the fact of me assisting and leaving the floor for the procedure."
"That is true." Dr. Bonner said suddenly looked thoughtful. He went out of the room, leaving Miaka with the doctors. "I'll handle it."
"You know your stuff." Dr. Miller said as she leaned against the table.
"Thanks." Miaka answered simply. Doing an act of checking her notes again to avoid the intense golden gaze intent on staring a hole right thru her.
Before Rafael could move to stand closer to Miaka and before anyone could ask her some more questions, Dr. Bonner returned with Jordan, the nurse lead and Jackie, the resource nurse.
"Miaka, let Jackie have your patients first, Dr. Bonner, want you to go to the ICU overflow with them in order to do this cardioversion. The manager knows about it too." Jordan said "Felicity is going with you so Tony will take over her patients"
"Ok" Miaka answered "excuse me while I prep the patient." As she turned to leave the room.
Once she was done prepping her patient, Miaka gave Jackie a quick but concise endorsement regarding her patients at the station. Once she was done she went to Mr. Inklers' room where Felicity was waiting for her. Handing her a pair of gloves and disposable gown.
"Are you ready?" Felicity asked.
"I guess" Miaka said putting on the gloves and donning the gown.
"Did you know?" Felicity asked her voice soft,
Miaka shook her head no.
Squeezing her hand, Felicity said "he has his reasons, but before you go thinking something else is awry, listen to what he has to say, alright."
Nodding her head and giving her friend a smile, Miaka went over to her patient's bedside to explain to her patient "Mr. Inklers, we're going to move you to the ICU, in order for Dr. Bonner to perform the cardioversion. If you have any questions before the procedure, you can address them to Dr. Bonner when we reach the ICU."
"Ok dear girl. I'm so lucky to have such great doctors and nurses taking care of me" the patient smiled
"Ok Sir, let's go" Felicity said as she started helping Miaka wheel the patient's bed out to the hallway.

Cardioversion is a medical procedure by which an abnormally fast heart rate(tachycardia) or cardiac arrhythmia is converted to a normal rhythm, using electricity or drugs. Synchronized electrical cardioversion uses a therapeutic dose ofelectric current to the heart, at a specific moment in the cardiac cycle.
