Derek was pacing in the emergency room waiting room by the time Hotch got there. The others he knew were on the way but had yet to arrive. Derek spotted him and he finally sank down in a chair. Hotch went to him and sat next to him.

"Have they told you anything?"

"We just got here and they took her to the back. They said they would come out and talk to me when they found out what was going on." He said. "She was bleeding so much."

"She had to have fallen from the stairs."

"I know but what I don't understand is that she was afraid to come down them by herself. That's why I told her to stay in bed until I came back. Something drew her out. Why was the door unlocked when I know I locked it?"

JJ and the rest came through the emergency room doors looking for them. Once they spotted hotch and Derek, they rushed to them.

"What happened?" Rossi asked.

"We had just come back from Derek's therapy when he went to unlock the door and realized it was unlocked. I drew my gun and went in first. We found Pen lying on the floor by the stairs and she was bleeding."

"She fell from the stairs?" Spence asked.

"It looks like it."

A doctor came out asking for the family of Penelope. "We are her family."

"I'm Dr. Abbott the obstetrician on call. I know you want answers but the only thing I can tell you is she is still bleeding and the babies are in distress. They are prepping her for an emergency C-section. Her doctor and a team of nurses and a pediatrician are getting ready as well."

"Is there something wrong with the twins?"

"They heart beat goes up and down. Dr. Addison would come out and talk to you but she didn't want to leave Ms. Garcia. She hasn't regained consciousness. Dr. Addison wanted me come and get you since you are supposed to be in the delivery room with her. We will be moving to the maternity floor where we will do the surgery. The nurse here will take you to put some scrubs on and then take you the OR."

Derek turned to his friends and didn't know what to say. "Go Derek. We will be in the waiting room. Everything will be fine. Go meet your sons." Hotch said.

Derek waiting outside the OR door waiting to be let in. He was nervous and worried. He prayed everyone would be ok. He was finally let in and the first thing he saw was Pen lying on the table. He looked around and saw the baby warmers a set of nurses standing at each one. Dr. Addison noticed him looking around.

"Derek come in and sit in that chair next to Penelope. Each baby will have a set of nurses to check him out and the pediatrician is here in case we run into some problems. We can't seem to stop the bleeding so we need to get the babies out to see what I'm dealing with. I will explain everything I'm doing step by step."

She told Derek exactly what she was doing. He kept his hand in Pen's, holding on tight and praying. What seemed like forever but really was only minutes he heard a cry.

"Baby A is here daddy and it's a boy." She said. "Would you like to cut the cord?"

With tears in his eyes, he cut the cord and gazed at his son, who was taken by the nurse. She placed him in the bed and then starting rubbing him. He cried the whole time until he was wrapped in a blanket and a hat put on to cover his head.

"Baby A weighed in at five pounds and six ounces, eighteen inches long." The nurse said and then Derek heard another cry but this was a little weaker than baby A.

"Baby B is here daddy and it's a boy."

She didn't bother to ask him to cut the cord, she just held out the scissors. He cut the cord and the nurse took him to the bed. She started rubbing him to keep him warm but his cry was getting fainter.

"We need the doctor over here." The nurse said.

The doctor was with the baby A went straight to baby B. She started checking him out and started issuing orders. He was barely going to ask what was wrong when Dr. Addison started shouting as well. The machine connected to Pen started beeping.

"We have a bleeder here. Someone get Mr. Morgan out of here. Now." She shouted.

Derek didn't know what was going on but he knew he didn't want to leave. It took two nurses to get out of the room and to the waiting area. One of the nurses looked at Hotch.

"There were complications in the delivery room. The doctor will be out as soon as she can."

She left and JJ and Emily guided Derek to a chair. JJ crouched down in front of.

"What happened Derek?"

"They're here, my sons." He whispered. "Something is wrong with Baby B and then something was wrong with Pen. They didn't want me in there."

"Did they tell you what was wrong?" hotch asked.

"No. His cry was weaker that baby A. Pen was bleeding and her machines went off."

Emily hugged him while everyone else looked at each other. JJ and Emily stayed on either side of him, silently showing their love and support. Rossi brought them all drinks while they waited for news. After about two hours, both doctors finally came out and Derek to follow them. He asked if his family could go with him and they said yes. They were shown in a room and everyone took a seat. Hotch sat on one said with JJ on the other. He looked pleadingly at the doctors.

"Let's start with the twins first. Baby A was born first and weighs five pounds and six ounces nineteen inches long. As of right now he is breathing on his own but he doesn't have the sucking instinct, which means he can't drink from a bottle. That is not unusual for twins. For that reason, he will need a feeding tube and he will also be in the NICU. Other than that he is doing fine. He's breathing fine and on his own. We'll keep an eye on him. He will have monitors connected to him so we can watch him. Now baby B weighs four pounds eleven ounces and eighteen inches long. He had trouble breathing right after birth so we have to put in a breathing tube, which is connected to a breathing machine." Derek let out a sob. "He also doesn't have the instinct to suck, so he also has a feeding tube. He also is connected to machines to help us monitor him and is in the NICU. We knew he was a little smaller than his brother. He is right next to his brother. You can see them as soon as we are done talking. Any questions?"

"How long will they be on the feeding tube"

"They will be in on the feeding tube until they learn to suck on a bottle or the breast. I couldn't tell you a specific time because all babies are different."

"How long will he have to be on the breathing machine?"

"Until he can breathe on his own."

He nodded. "How is Pen?"

"She lost a lot of blood and then when we were in the delivery room we couldn't stop the bleeding. I had no choice to do a complete hysterectomy to save her life. She also needed a blood transfusion."

"She won't be able to have any more children?" he whispered.

"No. I'm sorry Derek but her uterus was to damaged that is why she was bleeding. She is in a private room now but she still hasn't woken up. Sleep is good for her right now, it helps her body recover. She basically had two major surgeries; a C-section and a hysterectomy. You can see her now as well. Any questions?"

He shook his head no but JJ did. "Is she asleep because of her fall or the surgery?"

"She was responsive before we gave her anything before the surgery. She'll probably sleep most of the day. If you have any questions, please don't hesitate to ask them. I asked the nurses to page me as soon as she wakes up."

He shook hands with the doctors and told the team that he wanted to see Pen first. They all went to her room but he entered alone. Now it was his turn to cry when he saw her lying on the bed connected to machines. She looked so pale. He went to her, kissing her on the lips.

"Pen baby, our boys are here. I only saw them for a minute but I did see a lot of hair." He whispered. "You rest baby, I'll watch over our sons and you. I love you Pen."

He went back out and the girls said they would stay with Pen. The guys said they would go get dinner. Derek wanted to see his sons alone. He walked to the NICU which was on the same floor but on the opposite side of the floor. Derek picked up the phone outside the door told them who he was. The door opened and he scrubbed in like they told him to. He opened the other door and a nurse met him.

"Hi. I'm Rachel and I'm the twins nurse for the evening. I'll be here until eleven." She said. "There right this way. We have three levels in the NICU. Area one is for the babies that are about to be discharged to the regular nursery. Two is for babies that have minor issues and three is for babies that are really sick. The twins are in the corner because technically baby B is a level three but we didn't want to separate them. We have them were they can see each other and hear the other cry. "

She took him through some doors and right up to the incubators that held his sons. The tears automatically came when he glimpsed them connected to the machines. Rachel peeked at him.

"I know it's scary seeing this way but they are doing rather well. The machines help to lets us know something is wrong. They are each connected to an IV that is just giving them fluids. They are both connect to a heart monitor. The feeding tube through their nose is connected to their stomachs. They did tell you they don't have the sucking instinct. The bed keeps tell is what their body temp is. They did have their first bath before we connected to them. You can put your hand through the holes to touch them. You can also hold them. When they are premature we like to skin to skin contact. We will get a curtain out in net to you. You will take off your shirt or just open it and place the baby on your chest. Mom will do the same when she is able to come down. Do they have names?"

"Yes. Baby A name is Hunter Alexander Morgan and his brother is Noah Austin Morgan."

"Those are nice, strong names. I will put their names on a card and place them on the incubator. Would you like to hold them?"

"Can I?"

"Sure. Let me get a rocking chair." She left and came back quickly. He watched her open the incubator and take out Hunter. She handed him to Derek. "I know it looks like a lot of cords but don't pay attention to that. Support his head, just like that." She covered him with a blanket.

Derek held his son, blinking back tears. "Hi Hunter, I'm your daddy. I love you so much. Mommy loves you too but she is sick right now and can't come. Look at all that hair you have."

He touched his hair and then his eyebrows. Hunter squirmed just a little bit and Derek brought him closer to his chest, rocked. He spent thirty minutes holding Hunter before he asked the nurse if he could hold Noah. She repeated the process with Noah.

"Hi Noah, I'm your daddy. Mommy and me love you so much. We know you are strong and you are going to get better soon." The machine started beeping and Derek started to panic.

"It's ok daddy. One of his wires is loose." She started checking each one. "Here it is. He's fine. His numbers are good. Questions? Are you going to be spending the night with your wife?"

"I'm not going anywhere."

"Good. I give her room a call when it's time for the twins to bed fed and you can come and feed them."

"With a feeding tube?"

"Sure. There is a way to feed them. We fill a syringe with baby formula and you push through this little portal right here. We'll try a bottle first, then the tube."

"When do they get fed next?"

"At nine."

It was a little after seven. "I'll be back at nine."

He handed Noah to the nurse and took some pictures for Pen since he didn't know when she would be able to come see them. He kissed both his sons and left. He had been in the NICU for more than an hour. He found the guys in the waiting room.

"How are the boys?" Spence asked.

"They are doing ok. I got to hold them. They are so small."

"Do you have pictures?" Rossi asked.

"I do but I want Pen to see them before anybody else does."

"Understandable." Hotch said.

"I know you don't need this on top of everything else but I had a bad feeling something wasn't right. So I made a call to the police, a friend of mine, to check your house out."

"What did they find?"

"Your back door had been tampered with."

"What?"

"Someone broke into your house." Hotch said.

"There something else you should know." Spence said. "Your sister is here."

"How do you know?"

"The police cruiser that's driving by your house saw a car parked a down the street from your house. He got the license plate and called it in. It's a rental. So I called the rental place and it's rented to a Desiree Morgan." Hotch said.

"No. It's not possible. What are you telling me?"

"I'm not saying anything but I don't believe in coincidence." Rossi said.

Derek pulled his phone out and called his mother. she answered on the first ring. "Hello."

"Momma."

"Derek how are you?"

"Is Des there?"

"What?"

"Is Des there?"

"No. She said she was going to visit a friend."

"Where is this friend and where does she live?"

"Her friend Marlene from school and I believe she said Springfield. Why? What is going on?"

"Pen had an accident. The babies are here."

"What? What happened?"

"I don't have time to explain I have to go."

He hung up and looked at eh guys. "She couldn't have had anything to do with this." He whispered. He stared at floor for a few minutes. "Are the girls still with Pen?"

"Yes. Do you want to eat? We have sandwiches here."

"No thanks, I'm not hungry." He said standing up. "I'm going to spend the night here."

"Do you want one of us to stay here with you?"

"No. We'll be fine."

He walked to Pen's room, spoke with girls and they left. He sat down and couldn't wrap his head around that his own sister could have something do with Pen falling because he knew she fell from those stairs. He pulled up his chair next to her bed and took hold of her hand.

"I've seen our sons, princess. They are on the small side and a have a few problems but they are beautiful. A combination of both of us. They both a have a lot of hair just like you said they would. I can't wait for you to see what we made out of our love. They need you, mommy. We all need you but I know you need to rest. I love you Pen."

He spent most of the night going back and forth between rooms; feeding the twins and sleeping in Pen's room. It was about five in the morning when Pen's machines started beeping. He jumped up and noticed her hear rate was really low and so was her breathing. A team of nurses rushed into the room with a doctor right behind them. He was ushered out of the room and watched while they tried to stabilize her. His phone started ringing. He answered while staring in her room.

"Mr. Morgan, this Abby in the NICU. There are some complications with Noah, can you please come."

Derek hung up, took one last look at Pen and ran to the NICU. He went in and was met by Abby. She explained she was the charge nurse and told them they were working on stabilizing Noah. His heart rate dipped to low, setting off his machines. He couldn't see his son because he was surrounded by a medical team but he could hear crying. They had moved Hunter out of the way. Derek moved toward him and reached into the warmer to take his hand. He spoke softly to his son.

"It's ok little man, daddy's here. You're not alone. Your brother is going to be alright, so is your mother." he whispered with tears running down his face. "Please God, don't take them away from me, please."

He sighed when he heard Noah crying, it was still weak but he was crying. The doctor came toward him.

"Mr. Morgan, Noah's heart rate keeps dipping between the normal range and getting very low. I'm going to consult with a heart doctor that specializes in babies to come and look him over. We might need to run some tests to see if there is a problem with his heart. I'm not saying that there is but this is the second time his heart dipped but the first time his monitors went off."

"When will that doctor be here?"

"He should be here by eight this morning." She said. "Has your wife woken up yet?"

"No. Her machines were also going off when you called me." He said. "Is he ok now?"

"His heart rate is in the range it should be?"

"Spend a few minutes with him and then go check on your wife. He needs rest too."

He went up to the warmer and put his hand in, running it all over. "Daddy's here Noah. You'll be ok. I have to go check on your mommy but I'll be back as soon as I can."

He kissed both boys and walked back to Pen's room. The nurse was writing in her chart.

"What happened?"

"Her heart rate dipped really low. Her vitals have been good all this time but she's fine now." She said. "You received a call from the NICU."

"It was Noah. He might have some problems with his heart."

"I'm sure they won't find anything and he'll be fine. You will see. Call me of you need anything."

He nodded and just stared at Pen lying in the bed. This was supposed to be an exciting time for them and here she was lying in this bed, not even aware that she had their sons. He fell asleep for the first time since he found Pen lying in the floor. He wasn't aware when Hotch walked into the room. He put his hand on Derek's shoulder. He was startled awake and went outside the room to talk, he told everything that happened during the night.

"The doctor is going to be here at eight."

"That is what they said."

"It's eight-thirty now." He said looking at his watch. "What do you want me to do?"

"Find my sister and bring her here and make sure you bring the police."
