"What do you need, Sammy? What can I do?" (Dean wants to help, Dean needs to help.)
"Be afraid. You all taste so much better when you're afraid."
"Joke."
"What?"
"Joke about it… Please, Dean. Just...laugh about this."
They killed the son of a bitch (Sam killed the son of a bitch), but it went after him and the scars are there. (They'll always be there.)
"I'm your worst dream come true! I'm everything you ever were afraid of!"
"I need you to be able to laugh at this," Sam tells Dean as he trembles in his arms.
