His mind buzzing as it quickly cataloged the organs located on the left side of the body...and concluded without a doubt that Sam's spleen had ruptured.

AUish Tag to 9x16, "Blade Runners" – Hurt Sam / Big Brother Dean – Dean sighed harshly and turned in his chair, preparing to lay into his brother...and then abruptly changing his mind when he actually saw his brother. Because Sam was standing there...but he was barely on his feet as his blood covered everything.

Pre-9x17, "Mother's Little Helper" – Hurt Sam / Big Brother Dean / Sassy Abaddon – "I have something you don't have..." Abaddon singsonged over the phone, her voice eerily quiet as she taunted him. Dean froze, feeling dread crawl up his spine and suddenly wishing he knew where Sam was.

The Mark of Cain has a price, and it's not what Dean's expecting. But it may just be the thing to break through Winchester stubbornness. Spoilers through to 9.14, "Captives".

AUish Tag to "After School Special" – Teenchesters – Sick Sam / Big Brother Dean – There was a distinct line between feeling like you might throw up...and knowing without a doubt that you were definitely going to throw up. Such was the horrifying realization as Sam shifted uncomfortably and swallowed hard while he stood beside Dean in the hallway of Truman High.

Post 9x13 – Slightly Injured Sam/Big Brother Dean – Usually after a conversation like they had experienced the previous night, they would avoid each other, especially the next morning. But here Dean was. Because Sam needed painkillers and water and someone to call him on his BS...and all of that sounded like a job for Dean Winchester.

It's not just hunting that runs in the Winchester family, Sam is a shapeshifter with a canine form like his Dad was. Dean disapproves of pretty much everything, Castiel makes a point, and Gabriel is the only one who really understands. Sam discovers that it's possible to have what he wants without causing pain to anyone, including himself.

Gabriel is determined not to get caught up in the entire Apocalypse business. When Castiel shows up on his doorstep, begging him to play babysitter for a newly de-aged Sam and Dean, he changes his mind pretty quickly, though. There is no chance that this will turn out anything but hilarious after all, right?

It's bad enough to have visions of people dying, but experiencing it with them is a whole level of worse.
