Dean has died enough times to know when it's over. And here, with Cain, it's over. So when Dean wakes up again after being stabbed in the chest, he's surprised to find no trace of any wounds...and a twenty-three-year-old Sam standing over him in the hospital. Something has gone terribly wrong.
