Kara, kate, barry, other kara, and other kate all went to the location of the alert. Cisco stayed on comms and nobody knew where Alex was. They just figured that she was off doing her own thing. They knew she could handle herself. By the time they all breached to their spots, they all were getting used to everything. Barry being back in his suit without his speed, Kate having a doppelganger, kara in her old outfit in her human form, while her alien legendary creature was wearing the new super suit. Cisco having to fix this whole mess, the other Kate meeting a doppelganger of herself as a hero.
They were all just standing there waiting and trying to figure out what's happening at the same time. Except for the other kate who branched off to get into her position.

Long story short they can essentially manipulate your mind into thinking a life you are living is real when it's not. Kara explains.
