Meredith sat up from her position. She had enough of pregnancy, she couldn't even sit up in bed anymore. She was in the home stretch at 37 weeks. She definitely felt in it every way. Her belly was twice the size of either of her pregnancies which was expected but she didn't know how much more her body could stretch. She felt in every part of her body, the lack of sleep, the swollen ankles, her face filled out so much and she just felt exhausted. She looked over at Derek sleeping peacefully and she felt angry. Her hormones were angry, she wished she could sleep. She went on maternity leave last week because she couldn't walk for long before her feet turned into balloons. She looked around the dark room, cluttered was an understatement.

They had bought a dresser and a changing table that replaced a chair she had in the corner. Two bassinets were set up next to her side of the bed and baby clothes filled each of them. She needed to organize them, wash them, and put them away.

"Derek," she whispered as she shook him.

"Hmm?" He looked up at her. His tired eyes still shined bright.

"You alarm is about to go off," she whispered.

"Did you get any sleep?" He mumbled.

"No. I think your children are just preparing me for months of this," she huffed.

"Mhm, I'm sorry," he yawned, still laying down. He put his hand on her stomach and rubbed lightly.

"Are you taking CJ with you today?" She asked. He had taken him a few days to daycare just because it was so hard for her to move around and chase after him. The girls were out for summer break but they mainly occupied theirselves.

"I will if you want," he said as he sat up.

"Ok, thank you," she puckered her lips at him, he leaned over and placed a kiss on hers before he got out of bed.

"Any contractions?" Asked as he stumbled over the clutter.

"Nothing painful, just every now and then," she followed him to the bathroom in a waddle.

"They will come when they are ready," he smiled at her as he turned the water on in the shower.

"I know," she yawned. She was exhausted, she felt a warm trickle down her leg.

"What?" He noticed the shocked look on her face.

"I hate pregnancy," she huffed as quickly slid down her pants. She had a trickling down her leg.

"Did you just pee on yourself?" He chuckled as he was stepping into the shower.

"It's the water. I officially have had it with pregnancy, I heard the water and I couldn't stop it," she felt her cheeks turn red. She was utterly embarrassed. She had always kept some sort of bladder control but this pregnancy was completely different in every aspect.

"It happens," he shook his head with a laugh as he began to wash his hair.

"Derek, it's not stopping," she couldn't help but chuckle nervously. It was trickling out of her at a steady pace before it was like a pipe busted and a large gush came out.

"I think little dude is having too much fun on your bladder," he laughed, washing the shampoo from his hair.

"Derek, I don't think this is urine," she said honestly. She felt a small contraction and the water wasn't stopping.

"Do you think your water broke," he stopped and looked at her in shock.

"I think it did, I don't know. I've never had my water break on it's own," she shook her head and just laughed.

"Oh my god, I'll call Amelia," he quickly shut the water off, jumping out the shower and grabbing a towel. Meredith was in shock, she knew this day would come and had thought she had prepared herself for it. She felt scared, she was about to give birth and become a mother of 5. She got up and walked to the doorway, she looked over the clutter now the light had been turned on. There was so much that needed to be done it was unreal. She grabbed a basket and filled all the babies clothes into the basket and waddled to the laundry room. She started putting the clothes in the washer and turned it on.

"What are you doing?" Derek asked, he now had clothes on, his hair was still soaking wet.

"Derek we are not prepared at all for these babies. They don't even have clean clothes to come home to!" She explained.

"Meredith, it's a little late to be the worrying about that. We need to get ready," he said with a sense of urgency.

"It's not like they are popping out right this second. I'm not even having contractions consistently," she scoffed, bypassing him to go to their bedroom.

"Meredith, we can do all this when we get home," he sighed, shaking his head as she began to fold the baby blankets and put them away.

"You mean when I have stitches in my vagina and babies attached to my boobs?" She raised her brow at him.

"I'll take care of it, now we need to pack our bags," he grabbed his own suitcase and began putting some clothes inside.

"Derek, you are panicking. I've pushed two babies out of me. Scarlett's labor lasted eighteen hours and CJ's was almost twenty-four hours. We have time," she scoffed as she continued to straighten up the room.

"Mer, you're not being urgent enough. We don't know how far dilated you are," he said.

"Eh, probably a three," she shrugged.

"You aren't taking this serious."

"You're taking this too serious. We are both doctors, maybe I'll have a home birth," she shrugged.

"No. Not happening. If I have to drag you to that car, I will!" He raised his brow at her.

"Make me," she smirked. He shook his head, she just couldn't help. She loved watching him panic over little things.

"Meredith Grey," he said sternly.

"Derek Christopher Shepherd," she repeated the tone.

"I'm not joking anymore, Meredith," he raised his brow.

"Well then why aren't you making me?" She said.

"Alright, fine. I'll call Addison," he scoffed, shaking his head. She let out a small chuckle.

"Derek, I'm kidding. I'll get ready when I feel like it's time. I don't feel like it's time," she shrugged.

"This isn't a joking manor Meredith. You are in labor!"

"And the sky is blue. are we just going to state facts here?"

"You're being ridiculous," he shook his head.

"I know my body. Please just let me do this how I want," she sighed. She was feeling small contractions, nothing consistent.

"Ok, I'm going to finish my bag and I'll put them in the car. Let me know when you feel ready," he shook his head, he didn't want to fight with her. Not today.

Meredith finished putting away what she could, she put sheets in the mattresses that laid in the bassinets and she grabbed her suitcase. She put everything she knew she would need in there before she grabbed her phone. She called Cristina, she knew Cristina said she would be here but it was impossible now.

"Hey Cristina, I was just letting you know my water broke and I'm about to go to the hospital. Call me when you get this," Meredith said to the voicemail before hanging up. Cristina had been super busy and they hadn't spoken much. She decided to just let it go, she needed to pack her suitcase. She heard the front door and Amelia appeared in her doorway.

"What are you doing?" She looked at her like she was crazy.

"Amelia. I don't have consistent contractions and I feel ok. I'm not going to go to the hospital to sit around and wait," Meredith laughed.

"You could be dilating and not even know it," Amelia offered.

"We are going once I get this bag packed," Meredith said as she filed her clothes as she put them in the suitcase.

"Do you need help?" Amelia offered.

"I got it, can you just switch the clothes from the washer into the dryer when they are done?"

"Done. I'll clean up in here too," Amelia looked around the room.

"You don't have to," Meredith shook her head.

"I want to Mer, you're having my niece and nephew and you wouldn't let us throw you a baby shower. Least I can do is help out," Amelia shrugged.

"You are helping out, you are watching the kids while I push two babies out of my vagina," Meredith laughed.

"I'm cleaning whether you like it not," Amelia offered a smile.

"Ok, well can you carry this downstairs?" Meredith asked as she closed her suitcase.

"Of course," Amelia smiled as she took her suitcase and carried it down. Meredith followed slowly, she wasn't going to let them know the contractions had definitely picked up.

"You ready?" Meredith asked Derek who was sitting on the sofa.

"Let's go, thank you Amy," Derek said to his sister.

"You owe me one, just go and have my babies," Amelia chuckled as she walked them out the door.

"Have you thought about names yet?" Meredith asked Derek as she drove to the hospital, trying to keep her mind of the growingly painful contractions.

"Not at all, I want to see their faces first," he admitted.

"Ok," she nodded, she held her stomach as a contraction started up again.

"Are you-"

"Shh, don't talk," she mumbled. She had to release a small groan as she felt the wave come over her.

"Answered my question," he said as he pulled into the hospital. He parked as close as he could, coming around to help her out of the car.

"I've already called Addison, she's got our room ready," he said as he grabbed the bags from the back seat.

"Mhm," she nodded as she wobbled across the parking lot. As they got to the doors, Derek grabbed a wheelchair and helped Meredith on it.

"How can I help you two this morning?" The lady sitting at the front desk asked.

"I'm in labor," Meredith said.

"Oh that's fantastic, does your doctor know?" She replied.

"Yes," Derek spoke for her as he noticed she was having a contraction.

"Perfect Dr. Shepherd, I'll let them know you are on your way up," she smiled. She was super friendly as she followed them to the elevator. Derek wheeled her in and they went up to labor and delivery. They were met with Addison in her scrubs already.

"Perfect timing guys, I just delivered a baby thirty minutes ago. How you feeling Meredith?" Addison asked as they led her to her room.

"Pain. Give me the drugs," Meredith groaned as she held her stomach.

"Let's me check you first to see how well you are progressing and I'll let the anesthesiologist come up and get that taken care of. I'll let you get settled and put your gown on. Just hit the call button when you are ready," Addison said before she left the room.

"Do you need help?" Derek asked as he held out his hand to help her out of the wheelchair.

"I think I got it," she stood up and started undressing. She looked at herself in the mirror, she honestly didn't recognize her body. She was massive, her stomach stuck out twice as much as her other pregnancies and her stomach was covered in stretch marks. She grabbed the gown and had Derek tie it in the back. She pressed the call button as she sat down on the bed.

"Mmm, crap," she groaned as another contraction hit. It was like a roller coaster ride.

"Alright, let's check you," Addison came in and she put her gloves on before she checked her dilation.

"Your water is definitely broke, I feel some hair," Addison laughed.

"How far in me are you?" Meredith squeezed the side rail.

"You're 7cm dilated. Looks like we are getting very close to baby. I'll send the anesthesiologist in before it's too late for an epidural. The nurse is going to bring you some paperwork for you to sign if that's ok," Addison asked.

"Yeah, just bring me some drugs please," Meredith pleaded.

"Is there anything I can do?" Derek asked.

"Ice chips," she looked over at him.

"Got it," he smiled and kissed her forehead before hurrying out. She felt every muscle tense as the contractions were falling over her. The door opened and two nurses came in.

"Hi Dr. Grey," one smiled.

"Hey Kelly," she replied with a painful groan.

"I have some paperwork for you to fill out while nurse Margret hooks you up to the monitors to watch your contractions," she smiled. Derek came in just after them.

"Did I miss anything?" He asked.

"Nope, not the important stuff anyway," Kelly laughed.

"Alright just give me the pen," she sat up the best she could. She signed the papers, breathing heavily as another contraction rolled through her body.

"The anesthesiologist should be in here any second to give you that epidural, let us know if you need anything," the nurse took the papers from her with a smile and they left.

"How you feeling?" Derek asked. He quickly regretted his decision as she looked at him with a look that could kill.

"Like my body is trying to push a watermelon through a tiny hole," she mumbled and rolled her eyes at him.

The anesthesiologist came in with one of the nurses, he went over the procedure and helped her sit on the edge of the bed. Derek got the stool and pulled it in front of her. She put her hands on his shoulders, bracing herself. The five seconds of pain was worth not feeling the contractions anymore. It was over in a second before he helped her back laying down and gave her the button to control the pump.

"If you have any issues, let me know," he said.

"Thanks Ben," she nodded. He left as Lexie was coming in.

"How come no one called me!" Lexie said as she waddled herself in. She was 32 weeks herself but barely looked 27 weeks.

"Sorry, I just got here like an hour ago," Meredith said.

"How you feeling?" Lexie asked. She had her scrubs on and obviously working.

"Ok, this epidural works wonders," Meredith said with a smile. Her pain had went down significantly. She still felt the contractions but they were bearable.

"Good, I want to stay but I'm just taking a break right now. I'm suppose to be covering and watching Amelia's patients while she's watching the kids today," Lexie said.

"Oh it's fine," Meredith nodded, there was another knock and Addison entered with two nurses.

"I'm glad to see you look much better than when you came in," Addison laughed.

"Thanks for the drugs," she joked.

"Anytime. I'm going to check you again just to see how far you've progressed," Addison said before she did. Her eyes looked almost shocked.

"You are at a 10 Meredith, are you ready to have some babies?" Addison said as she removed her hand and took her gloves off.

" what?" Meredith's jaw dropped.

"Oh I'm not leaving now," Lexie smiled like a child in a candy store.

"We're having the babies?" Derek's eyes widened.

"Looks that way. I'm going to get everything ready while Kelly and Margret here help you do some practice pushes to get baby A into position," Addison said and things went quickly from there. Lexie moved to her right side while Derek was quick to jump into action.

"You're going to do amazing," Derek said, he had a tear in his eye as he grabbed her hand.

"I love you," she smiled up at him. The nurses helped her move up on the bed to release the bottom half. The put her feet into the stirrups and started getting her preped.

"Alright Meredith, with this next contraction I need you to give me a big push," Kelly said as she was at the end of the bed. Meredith felt the next contraction and she began pushing, grabbing the bottom of her thighs with both hands.

"That's it Meredith," Kelly said. She stopped as Margret had finished counting to 10.

"Wonderful, if you keep pushing like that, you'll pop these babies out no problem," Kelly smiled up at her.

"You can do it Mer," Lexie cheered as she watched from the side in awe.

"You got this," Derek was leaned down in her ear, brushing her hair back as she was resting.

"Alright here comes another one, big lush," Kelly instructed as she watched the monitor. Meredith pushed with all her might. She let out a loud yell as she released something inside her.

"Good good, I can see baby's head!" Kelly said again.

"You're doing amazing Mer," Derek kissed her forehead again.

"Alright, who's ready to have some babies?" Addison asked as got into position. Both nurses then grabbed her legs as they waited for the next contraction.

"I can't believe this," Lexie said in excitement. She hadn't been able to be there for both births because she was working at the time.

"Alright here we go, Mer when I say go I need you to push like you haven't pooped in three weeks," Addison said.

"I don't want to poop!" Meredith said quickly.

"It's a good thing, it means you're doing it right. Alright 3 2 1, push!" Addison said. Both nurses pushed her legs back in and Derek couldn't help but watch in amazement as he saw the baby's head come out.

"Oh my god!" Meredith yelled loudly as she couldn't hold it any longer.

"His head is out Mer!" Derek said in awe.

"He's got a head full of hair," Addison said.

"It's dark," Derek smiled as he looked up at her. She looked absolutely exhausted, she was drenched with sweat, her body glowed under the lights.

"Alright one more push to get the shoulders out," Addison said as she counted down and Meredith gave it her all. She closed her eyes in focus. She was pushing as hard as she could.

"He's out!" Derek choked as he said it. Meredith eyes shot open as Addison held up the tiny boy.

"Awe," Meredith couldn't help but burst into tears of joy. Addison quickly laid him against Meredith's chest.

"He's perfect," Derek whispered in her ear, kissing the side of her head multiple times. He was also crying.

"Alright Mer, it's time to push the placenta out," Addison instructed. She nodded, she was too focused on the baby boy nestled to her chest. She held him close, his bright blue eyes shined up at her and she couldn't help but kiss his head. He looked a lot like Derek with her cheeks. She closed her eyes and breathed as she pushed the placenta out. Addison quickly clamped and cut his cord before pulling the placenta out as well. She put it in the waste bag that held all of Meredith's fluids under her.

"I need to check him out real quick and I'll give him right back," Kelly said as she took him from her chest. He immediately began to scream and flail his arms.

"Awww," Meredith breathed as she watched her take him over to the bassinet. Lexie quickly followed the nurse over as she watched her nephew get his apgar test done.

"Ok Meredith, now it's time to get your little girl out," Addison said. Meredith felt exhausted, she honestly didn't think she could do it.

"Ok," she nodded. It was a quick minute before another contraction rolled and she was pushing again.

"Ok, ok Meredith stop," Addison said quickly. She grabbed the babies head as it came out.

"Is there something wrong?" Meredith looked terrified.

"Just the cord, let me just-" Addison said as she pulled the cord from around her neck gently.

"Ok, you can push when you're ready," Addison said. Meredith felt a contraction and was quick to push with everything she had.

"Mer, she's beautiful!" Derek said as Addison pulled her out and up to show her off. She looked smaller than the boy which they expected.

"She's perfect," Meredith whispered as Addison laid her on her chest.

"She looks like you," Meredith said as she ran her hand over the girls dark hair. She didn't have nearly as much as the boy but there was enough there to tell it was Derek's color.

"She's amazing," he whispered, he leaned in and kissed Meredith.

"Congratulations guys, I'm just going to stitch you up," Addison said as she grabbed the suture kit.

"Alright, let's trade," Kelly said as she brought over the boy.

"How is he?" She asked.

"He's a perfect 6lbs and 18 inches long," she noted.

"He's so little," Meredith held the baby against her chest. He started nestling down against her breast.

"Here," Derek said as he helped her unbutton the shoulder on her gown and pulling it down. She positioned him and he quickly latched on to her breast.

"Hungry little guy, aren't you?" Meredith giggled as she ran her finger across his cheek.

"He's a master latcher," Derek joked. He watched in amazement as he quickly suckled what he could from her.

"Say hello to your little girl," Kelly smiled as she brought over the girl and handed her over to Derek. He had tears rolling down his cheeks as he held her tightly.

"How big was she?" Meredith asked.

"5lbs 11oz and 17 inches long. Just a little behind her brother," she smiled.

"Meredith, they are perfect," Lexie said in awe. She had also been crying with the rest of them.

"Thanks Lex, I'm glad you finally got to see one of my birth's," Meredith chuckled.

"I hate to run, I want to get my hands on these babies when I get back!" Lexie said as she left the room.

"Alright, we are going to leave you guys alone and let you bond as a little family. I'll be back here in an hour to move you guys to mother and baby," Addison said as she left the room with the nurses.

"Here, let's switch so she can get a chance to eat before her brother takes it all," Derek laughed. It was a little tricky but they managed to switch them easily.

"We had twins," Meredith whispered as she watched her daughter latch on.

"It's crazy," he stood beside her, slowly bouncing the boy in his arms. He looked at peace with the world and like they were in their own little baby bubble.

"Have you thought of a name yet? I know we just met them," She asked as she looked over at him quietly holding the baby boy. He contently laid in his arms, looking up at Derek. He was a good mix of the two of them.

"This is such a hard decision. I mean I want his name to be strong and meaningful," he said.

"Yeah. I was thinking about her name," she smiled as she looked at the girl in her arms. She was suckling away like a pro. She looked so much like Derek it was uncanny.

"Let me hear it," he smiled over at her. He was in a complete state of awe and serenity.

"Given the past year, and how my life has been turned upside down, I just couldn't stop thinking about my mother. I know it's strange, I never talk about her but she was the strongest woman I know. She never let anyone hold her back from her dreams and she had been through so much so I really wanted to name her after my mother."

"Ellis?" He said.

"Well, I don't really like the name Ellis. So I was thinking more girly. Elliana," she smiled.

"It's beautiful," his eyes sparkled as he looked at her and the baby.

"Elliana Caroline," she finished, "Caroline is Lexie's middle name and it also is for your mom."

"I love it," he leaned down and kissed her forehead.

"You know, I really want to name him after me but also after my father," Derek slowly bounced the boy in his arms.

"Christopher?" She asked.

"Christoper Elliot," he finished.

"Aw, but can we call him Elliot?" She asked.

"Yes," he nodded.

"So we have little Elliana, and Elliot," she giggled.

"Our perfect family," he leaned down and kissed her. It was true, she knew her family was complete. She had the perfect boyfriend and perfect children. She couldn't ask for a better life.
