19 hours! 19 hours Derek was asleep for on the bathroom floor. It was 2pm the next day when he started to wake up. Meredith, Mark and Addison had taken it in turns to stay with him while the other could sleep or get something to eat. Nancy and Lizzie helped too but not that much. Derek fluttered open his tired eyes and rolled onto his sore back. Sleeping on the bathroom floor was far from luxurious. Addison was with him,

"Your awake!" Addison smiled.

"Hmmm..."

"You've been asleep for 19 hours!"

"Really?"

"It's 2pm, you fell asleep at 7pm last night."

"Wow."

Addison helped him sit up against the wall, he felt useless and trapped. Derek quickly stood up felling less exhausted, the pancreatitis must've started to subside he thought. He walked into his bedroom and shut the door behind him leaving Addison shocked. He quickly changed into a pair of shorts, a T-shirt and a pair of running trainers. He then grabbed the bottle of juice from his bed stand and opened the door where Meredith was stood waiting for him.

"Derek?"

"Yes?"

"What are you doing?" She asked grabbing his arm.

"I'm going running."

"Derek you can't do that!"

"Well I am." He said coldly.

Before Meredith could even reply he had walked down the stairs and out of the door. Meredith knew someone had to run after him so she went to get Mark downstairs. As soon as Derek left the warm house he felt the cold summer breeze of Seattle on his skin.

He started off at quick pace, feeling his body's protests almost immediately, but he didn't stop. If anything, he picked up his pace just a little more, suddenly he got the urge to push himself slightly over his limits. It was getting harder and harder to breath. His muscles were tired from the lack of food and the illnesses compromising his weak immune system, his left side, right bellow the rib cage felt like it was on fire and he could feel his heart beating faster and faster. But he just pushed through the pain and continued on. After an hour he decided he should probably turn back in the direction of the house even though he was exhausted. He thought about calling someone to get him but he didn't. His pace had slowed down and another hour later he arrived back at the house.

Derek knew it was a bad idea to go out for a run. By the time he finally got back he couldn't catch his breath, his vision was beginning to grey at the edges and he felt so nauseated he was sure he was going to throw up any minute. He fumbled with his keys and he was quite shocked when he actually got the key in, in only one try, when he was shaking so badly. As he stepped inside he was stricken with a sudden wave of dizziness and he half leant half fell back against the wall. He could feel bile rise at the back of his throat and he shut his eyes and swallowed hard. The last thing he wanted was to vomit all over the floor. The dizziness suddenly intensified, his knees buckled and he slid to the floor. Yeah, he knew it was a very bad idea.

He heard someone call his name but it sounded so far away and he felt too weak to answer. He felt a light pressure at his right wrist. After a few seconds he heard the same voices calling for him again. Closer this time. Meredith, Mark, Addison and his mom.

"I don't feel s'good" He slurred.

"Hear, drink this."

And suddenly a glass was pressed against his pasty lips. He took a small sip and coughed a little.

"Drink some more."

The voice ordered him and he did so. The dizziness subsided as well as the nausea. He opened his eyes only to see everyone's concerned eyes looking straight at him.

"Derek." Meredith sighed.

"Is that any better?"

"Yeah, I feel better" He answers.

"You think you can make it upstairs?" Mark asked.

He nodded. He placed his right hand against the wall and pushed himself off the floor and was surprised when Meredith and Mark grabbed his hands and helped him up. He swayed a little and Meredith and Mark grabbed his arms to steady him. They were almost at the top of the stairs when he felt sick again. He covered his mouth with his hand and dashed for the bathroom. This time he couldn't hold it in anymore and he violently vomited, hands clutching the toilet bowl. He had barely had anything to eat the past few days that he hadn't thrown up due to the pancreatitis so he was only vomiting bile. The acid burned his throat. He felt Meredith's resting a hand on his back slowly running soothing circles. She filled a glass of water with her other hand and handed it to him. He wiped his mouth with one hand and grabbed the glass with the other.

"Thanks"

He took slow large sips whilst trying to get his breath back.

"Why don't you have a shower?" Meredith asked soothingly.

"Sure." Sighed Derek.

He stood up and walked the few steps to the shower. He turned on the shower and let the water run for while to warm up as he undressed with Meredith's help. He stepped inside and felt his muscles relax under the hot water. It felt nice and he felt clean. Meredith stood outside with the others as he showered. His sisters had returned to the hotel to catch up on their work. He finished a few minutes later and reached out for his towel that Meredith had put outside as well as some pyjamas. He was exhausted again but he knew he was about to become even more when the chemo started. Derek got changed into the pyjamas and looked at himself in the mirror. His hair was dripping wet and flat to his forehead, his skin was pale, black bags engulfed the bottoms of his blue eyes and his lips were almost as pale as his skin. He rubbed his hand over his stomach wincing in pain every time he breathed. After a few deep breath's he walked back out of the door where they were all waiting.

"Derek, Dr Swender and Dr Hunt is downstairs." Meredith said grabbing onto his arm.

"What?"

"I don't know, they said that they wanted to come check on you because you hadn't been returning their calls."

"I didn't know they'd called." Derek sighed.

"Come on dear." Carolyn said.

As soon as he stepped off the staircase the dizziness from before returned worse.

"Mer...dizzy." He said grabbing hold of the bannister.

Before Meredith could respond Derek fainted to the floor! Meredith screamed for Dr Hunt and Dr Swender and they came running towards him.

"What the hell?" Hunt cursed.

Owen and Mark quickly placed him in the rescue position and Dr Swender checked his pulse and temperature.

"What has he done today?" Rebecca asked.

They all looked at each other knowing that he fainted because of the run.

"What happened?" Dr Swender said standing up.

"Well...he is struggling a lot because he can't do everything he used to be able to do. He was asleep for 19 hours straight but when he woke up a few hours ago he stubbornly went running." Addison said sheepishly.

"What? How long was he outside for?"

"2 hours, we couldn't find him." Meredith said.

"We need to get him to the hospital, I'm calling 911."

Just then Derek woke up and vomited all over himself. He was barely conscious and his temp was at 103.9.

"Meredith dear, have you already packed a suitcase for him?"

"Yes, let me go get it."

Meredith shot up the stairs while everyone else got on their coats and shoes and put a jacket on Derek.

"The ambulance is on its way, 5 minutes." Dr Swender said walking back into the room.

"Why is this happening?" Carolyn said as she sat down next to her struggling son.

"I think that being out in the cold earlier with his compromised immune system has caused a sort of pneumonia but mixed with the pancreatitis he already has this could become serious. I want to get him in today so we can run his tests before he starts chemo tomorrow morning." Dr Swender said.

Meredith ran back down the stairs with a medium sized brown suitcase, a small back pack and her purse.

"Is that everything dear?"

"Yes."

"Me and Addison will take it with us now, we're going to meet you there, in the ER?"

"Okay Mom."

"See you there." Carolyn said needing to leave, she couldn't bear to see her son like that.

5 minutes later the EMT's knocked on the door and Hunt went to let them in.

"Where is he?" One of them said pushing in a stretcher.

"Just over here."

They walked over to him and started checking the unconscious Derek's vitals.

"His pulse is weak and thready, pulse ox is low, what happened?"

"He has early stage acute promyelocytic leukaemia, acute pancreatitis and I think he may have pneumonia." Dr Swender said.

"What treatment is he on?"

"He hasn't started yet, he is going to start Hyper-CVAD chemo tomorrow morning."

"Okay, let's get him on now."

The 2 EMT's and Mark lifted him onto the stretcher. They all left the house, Meredith quickly locked it and they got in the ambulance. Mark, Meredith and Owen sat in the back with him and Dr Swender sat in the front. It took 20 minutes for them to arrive at the hospital.

The doors flung open but Derek was still unconscious.

"42 year old male..."

"It's Derek!" Owen shouted to Bailey, Richard, Teddy, Cristina and Karev.

"What the hell?" Bailey cursed.

"Possible pneumonia and he already has acute pancreatitis and acute promyelocytic leukaemia." Dr Swender announced.

"Pulse is at 57, pulse ox is at 92% and temp is at 103.7." The paramedic said wheeling him out of the stretcher.

"Trauma 1." Richard said.

They swiftly took him into the trauma room and transferred him onto the gurney. The paramedics left and they all started to work away on him.

"Mark, Meredith." Addison said holding onto their shoulders.

"Yes?" Meredith said hardly concentrating.

"You need to come with me, Mom is next to the front desk we need to wait for him there."

"But-"

"No come on." Addison said.

They all walked out of the room not taking their eyes off of them.

"Okay, start a bag of warmed saline, 10L of high flow oxygen and 5mg of morphine and 400mg of Tylenol." Teddy shouted.

"I need a full CBC, white count, red count and platelets stat." Dr Swender said as she gave him oxygen.

"We need him conscious, Karev eat the smelling salts." Owen said.

Karev did as instructed and gave it to Hunt. He removed his oxygen mask and placed it under his nose. After a few seconds he woke up panting for air so Owen placed the mask back on him.

"We need a chest X-ray." Teddy said after checking his lungs.

A man went into the room with a portable X-ray machine. They all stepped away from the table and took the picture.

"I'll send the films through in a minute." The man said leaving again.

"I have the blood test results, Cristina told me to give them you." Lexie announced walking through the door.

She didn't know anything about Derek now, she knew he had cancer but he was her mentor and seeing him like that almost hurt her.

"What...what happened?"

"Out Grey." Teddy shouted knowing how she is technically family.

"His white count and platelets are seriously low, hang a bag of O neg and platelets."

"Done." Cristina said.

"Are the bruises normal Dr Swender?" Richard asked.

"Yes, how many does he have?"

"7, 7 large ones."

"Okay."

Derek had slipped back out of consciousness because he was hurting so much. 10 minutes later he was finally stable but had wires and IVs everywhere.

"Dr Swender do you want us to place his Hickman line here?"

"Yes please."

So they did, they placed his line above his left breast, it was just an IV port so he wouldn't be stabbed every time he had a session.

"Done, are we taking him up to the cancer unit?"

"Yes, I'll meet you up there I need to talk to Dr Grey, Dr Sloan, Dr Montgomery and his mother."

They left the trauma bay and Dr Swender walked out to them.

"How is he?"

"He's stable, he doesn't have pancreatitis anymore but he has minor pneumonia. It can be managed with some meds and he is fine to start the chemo as long as he eats tea and breakfast as he has hardly eaten."

"Okay, can we go and see him?"

"Yes follow me."

They went up to the 5th floor where the cancer unit was. Teddy, Owen and Karev were all waiting outside of his room watching him with a universal shocked expression plastered over their faces. They walked into the room to see Derek softly crying, the tears rolled down his pale face and his shoulders shaking. Meredith went straight up to him and kissed him on the forehead.

"Oh Der..."

"I...I'm useless." He cried.

"No your not, your sick, you look after everyone and you have since you were little. It is our turn to look after you."

"No...I don't want that to have...to happen. I can't...even walk or sit up on...my own."

"Derek, that is expected. Your sick, really sick and your body is trying its hardest to fight back but unless you rest and eat it can't do that." Dr Swender said.

Derek didn't reply but sighed quietly. Meredith sat down next to him avoiding touching any wires or IVs. She stroked his hair and held his hand tightly. When it was appropriate, Dr Swender continued talking once he finished crying.

"To start chemo tomorrow, you are required to eat tea and breakfast. I am hoping that now your pancreatitis has gone you will be able to keep it down."

"Oh." Sighed Derek.

"I'm going to go get it you now." She said leaving.

Meredith sat down next to him once again and hugged him trying not to hit all the wires.

"Thank you Mer."

"I love you." She said kissing him on the forehead.

"I love you more."

"Feeling any better?"

"A little, my chest doesn't hurt as much."

"Oh good."

Mark burst into the room panting and crying.

"There's been an accident..."
