"Oh my God Cailin. Really? Your mom is going to kill you." Jess quickly tried to reel in her laughter as her mom shot her that look that told her she was skating on thin ice.

"Only if I don't kill her first," Maura said. Maura was not amused in the least. "Cailin, get your ass in that bed now."

Maura goes to swat her sister on the butt to get her moving and as she did she quickly stopped as her eyes widened.

"Cailin, please tell me you didn't walk all the way from the hospital to here with your ass shining to the entire world?"

At that, Jane and Jess lost it all over again. In fact, Jess laughed so hard she began to have tears running down her face. Soon, Maura gave up the fight and joined them.

Cailin groaned as she climbed into the bed, her face reddened in embarrassment.

"Really Cailin, not even a pair of underwear?" Maura shook her head at her sister.

"I couldn't find what mom did with my bag. It's not like I realized it came untied."

Maura shook her head again. She couldn't believe her sister had walked two miles pushing an IV pole with meds attached and pulling an oxygen tank. If that wasn't enough, she did it with her ass shining to all the traffic coming and going.

"I'll be right back. I'm going to get my bag so I can check you out. Don't you move a muscle."

After Maura left, Jess and Cailin both looked at Jane. Finally, Jess spoke.

"Ma, would it be OK if I talk to Cailin alone for a few minutes?"

Jane thought about it for a moment. "I probably shouldn't considering what Cailin pulled but since she gave me a good belly laugh, I'll give you five minutes." Jane turns to walk towards the door before looking behind her at Cailin. "You better be prepared Cailin. You're going to be the butt of my jokes for a long time to come."

Cailin simultaneously groaned while throwing a pillow at the brunette, narrowly missing her as she made her escape out the door.

After Jane was gone, Cailin turned her attention to Jess. "OK Jess. I know something is up. Jane tells me you are helping Blaire at the Boys and Girls Club while Maura turns around and tells me you're sick. I'm not dumb. What is really going on?"

When Jess remained silent Cailin spoke her suspicions out loud. "You have been using again haven't you?"

Jess took a deep breath before she took Cailin by the hand. "Yes Cailin, I have." Jess's eyes filled with tears before she continued. "There's more. All I ask is please don't leave me Cailin. I'm getting help."

Cailin nodded. "As long as you are getting help Jess."

Jess curled up beside Cailin on the bed and pulled her close to her.The two laid there in silence before Jess dropped the bombshell. "I was taking Heroin Cailin."

Cailin's eyes flew open as she sat up and looked at Jess. "Shit Jess, why?"

Jess pushed herself up and sat on the edge of her bed. "I haven't handled everything that's happened very well."

"Yeah no shit Jess." Cailin snapped back but instantly regretted it when hurt and sadness washed over her girlfriend's face.

"Don't hate me baby, please. I know that is selfish of me to ask but I couldn't handle you hating me. I'm going to get help and get better but for real this time."

As Jess looked at Cailin she could see the words that she was not saying. "This is not your fault Cailin. Yes, you being sick has been hard on me, but there is a lot of unresolved stuff from my past I'm having to work through. Please don't blame yourself."

"Jess, I love you but if you don't choose us over drugs then i'm going to have to break up with you. I can't handle that Jess. I just can't. I won't watch you kill yourself."

"I'm done with it Cailin. I swear to you. I don't need it. You're what I need. Please stay with me through this. I promised to fight baby and I am. I want to be my best self not just for my family and myself but for you. When the day comes that I ask you to marry me I don't want you to have a single doubt, I love you C, please don't go."

Cailin nodded and burrowed herself into Jess's arms.

The sweet moment was short lived as Hope came charging through the door madder than a nest of stirred up hornets with Maura trailing behind her.

"Cailin Jade Martin, what in the hell is the matter with you? I'm not believing you took off like that. You're in your third year of medical school. You know how sick you are. There is absolutely no excuse for you doing something so careless and stupid! "

Jess got a mental image of Cailin walking down the side of the highway pushing an I.V. pole while lugging an oxygen tank behind her and couldn't help herself as she broke into laughter again. It was obvious her girlfriend loved her. Really loved her.

Jess continued to laugh to the point her stomach was hurting and tears were running down her face for the second time that day. Suddenly, she was shaken from her fit of laughter by her mom's no mess voice.

"Jessica Lynn Rizzoli Isles, that is enough. Knock it off. This is not funny in the least."

Maura made her way over to her sister and opened her medical bag and started checking her out while Jess stared at Maura with her mouth open before a huge grin filled her face.

"You both have always just said Jessica Lynn when you were yelling at me. You've never used my last name at all, or yours."

Maura stopped her movements as she thought about what she had just said. She couldn't help but smile. "I told you Jess, you are our daughter." Maura spent some time checking out her sister before speaking.

"Your oxygen level has dropped, no doubt from your little stroll. I'm going to turn this up," Maura said as she reached over and adjusted the oxygen tank.

"Mom is going to call an ambulance to take you back to the hospital. You see Jess is fine and we will bring her to see you. She's going to head to rehab soon though."

Cailin's eyes filled with tears. "Please don't make me go back."

"You have to Cailin. You know you are still very sick. Don't you want to get better and stronger so you can come home?"

"Yes, but why should I have to go back to the hospital? Two of the best doctors I know are standing right here in this room. I don't need no stinking hospital. I want to be home with my family."

Maura laughed. "If that's your attempt at buttering me up, it's working but i'm still mad at you. If you insisted on busting out you could have at least called me so I could come get you."

"Would you though?"

"Well, no but I would have if you told me you were going to walk two miles down the highway."

"So can I stay? Please?" Maura looked from Cailin to Hope. She knew ultimately it wasn't her choice to make.

Hope shrugged her shoulders. "What are you looking at me for? At the end of the day she is grown and she seems to have somehow inherited Jane's stubborn streak without even being biologically related to her."

Maura nodded. "Okay. Mom will call them and tell them you went Awol. If any of your stats drop too much it's right back to the hospital though. Is that understood? We are not equipped to handle a serious emergency."

Cailin nodded. "Yes ma'am."

Maura groaned. "Cailin, how many times have I told you don't call me ma'am."

Jess looked from Maura to Cailin and back again. "Wait a minute? What? Why is she special?"

"Because I'll tell you for the millionth time today Jess, you are my child. For you it is a requirement," Maura says as she winks at the redhead.

Everyone laughed as Cailin's cell phone started ringing and she picked it up.

"Hey Jane." She listened for a moment before grinning.

"Yeah, it's safe for you to come back over." Cailin listened for a bit longer before her eyes grew wide.

"Wait, What? No way Jane. You're just trying to punk me." Cailin's face started to turn redder by the second."OK. See you in a minute."

Without a word she pulled up her Facebook on her phone and went to the page Jane told her to go to. That was when she realized that Jane was in fact not punking her. She blushed as she slammed her phone down on the bed beside her and groaned.

"Cailin what's wrong? What did ma say?" Jess asked, concerned. Cailin just groaned again.

"C'mon C, what's going on?" Cailin picked up her phone off the bed and showed Jess.

Jess did her best not to immediately laugh but failed. "Oh no Cailin, babe, I'm sorry. I don't mean to laugh," she said quickly, seeing the glare on her girlfriend's face.

Maura and Hope looked at each other in confusion before Maura asked what was so funny. Jess went to hand the phone to Maura but Cailin snatched it away.

"Nope, having Jane see it and you and lord knows who else is humiliating enough," Cailin remarked, burying her face in her hands.

"Come on babe. It's nothing your mom hasn't seen before and my mom is a doctor so…"Jess was trying her hardest to make this better but seemed to only make it worse. Finally, Cailin relented and showed Maura her phone. Hope leaned over to see as well.

Both women began to chuckle. "Oh Cailin," Maura said, trying to hold it together. She really didn't want to make her sister feel any worse.

"Yeah sure, all of you just laugh it up. I mean, it's not your literal ass being plastered on Facebook." Cailin's comment didn't make the matter at hand any less funny.

Jess, feeling bad for her clearly embarrassed girlfriend again tried to sooth her. "C it's okay, no one knows it's you. It's a picture from behind."

Maura and Hope both broke into laughter again.

"Jess, please stop. That's not helping."

"I wasn't making a joke, I swear. I was only saying you can't even see your face babe, so as far as everyone else is concerned it's just some random person walking down the highway." Jess's statement seemed to help a little as Cailin rested her head on her shoulder.

"Can you all promise not to show or say anything to anyone about this, please?" Cailin asked as she looked from her sister to her mom who both nodded in agreement. "Thank you."

Just as everyone was moving past the embarrassing photo, Jane came in.

"What's up sweet cheeks?" Jane grinned at Cailin mischievously who buried her head in her hands and groaned once more.

"Seriously Cailin, if you wanted some fresh air all you had to do was call. I could have taken you for a couple of laps around the hospital and made sure I covered your ass. You know I always have your back." Jane and Jess both burst into laughter.

"It's not funny, stop it," Cailin whined.

Hope looked at Cailin and grinned.

"You don't have any reason to be embarrassed honey. I happen to think that is one of the cutest little hineys around. I diapered that butt."

As if everyone wasn't harassing her enough, Blaire came charging in, phone in hand.

"Man C, you just busted out not even an hour ago and this already has 10,000 views. You couldn't be bothered to put a pair of pants on before you busted out of the joint? At least a pair of underwear," Blaire said with a laugh.

As Riley walked in Cailin was done. "Not one word," she said as Ry walked through the door.

Riley held up her hands. "Hey, I wasn't going to harass you. Em and I are just heading down to the cafe to get lunch and I was going to see if you wanted an order of sticky buns?"

With that one, everyone in the room broke into laughter, even Cailin.

"I am so glad I could be your morning entertainment everyone. I'm going to write a complaint letter to the hospital about those gowns. If they buttoned instead of tied this wouldn't have been a problem."

Jane grinned before walking towards Riley. "I'll go stay with the littles so you guys can go get lunch. Thanks for doing that. I'm starving."

"You're always starving," everyone in the room said in unison.

After Jane left and everything had settled, Jess looked over to Maura and Hope.

"I was wondering, um, would it be okay if Cailin and I were alone until everyone comes over for lunch. I mean, you can stay nearby or whatever. I just need to talk to her privately for a bit longer if I can."

Maura and Hope both had a feeling they knew why she wanted to talk to Cailin so they both nodded in agreement. After they were gone, Jess turned to Cailin.

"Listen C, I wish I could wait to tell you this but I don't really want to feel like I am keeping it from your so I am just going to come out with it."

Cailin nodded as she sat up in bed and looked at Jess intently.

If Jess thought telling Jane and Maura was hard, that was easy compared to this. "Okay, so you know when your mom and I had that big fight?"

Cailin nodded. "If you're upset about her being at the hospital with me I…"

Jess held up her hand before Cailin could continue. "Cailin, it's fine. Especially when you hear what I have to say."

Jess inhaled a deep breath. It was now or never and she hoped that this wasn't going to destroy everything.

"That day when I came to see you, your mom actually was going to let me see you. We were standing at the front door talking and a syringe filled with heroin fell out of my purse when my purse shifted the wrong way. Your mom kind of freaked out. She screamed at me that I was just a junkie and that I didn't deserve you."

Jess paused as she started to cry. "I can't say I really blame her under the circumstances. If the roles were reversed ma would probably freak out as well. The thing is though, I was so scared. I was hooked and I couldn't imagine stopping. So, so I lied. I told ma that Hope said that they never should have taken me in and that they should have let me die. That didn't really happen though. I just knew she would tell mom. I knew if mom thought that happened she would cut your mom off and my secret would be safe.I am so ashamed of what I did and I wish I could take it back. I realize I hurt mom, Kate, you, and Hope all four. I'm especially sorry that I lied to you. Like I said, I wish I could take it back more than I've ever wanted anything. I can't though."

Cailin stared at Jess for a moment before anger crossed her face. "Mom was telling me the truth all along. I thought, there has to be more to this. So I called Maura and I asked her and I got your fucked up version of events. I have cancer Jess. I damn near died. You could have cost me the last days I had with my mother."

Jess went to take Cailin's hand but she quickly snatched it away. "Don't touch me."

Cailin sat for a second before she spoke again."Get out."

Jess tried but failed to hold back her tears. "Cailin, I,"

"Leave!" Cailin yelled, her own tears falling freely.

"I can't leave. Mom is not going to let me." Jess replied, fighting the urge to run.

"Well, I guess they need to figure something out because I don't even want to look at you right now."

Without another word, Jess silently got up and walked out and straight to Maura who was nearby. She began to sob as she collapsed into her mother's arms. Maura didn't even have to ask what happened. She knew. She had no words. All she could do was hold her daughter while she cried.

When Jess managed to semi pull herself together, she spoke. "S-s-he s-said I have to l-leave."

Maura shook her head."No, you're not going anywhere. You are still every bit as sick as she is right now. She's just going to have to deal if she wants to stay here."

Maura kissed Jess on the head before stepping away. "I'm going to go talk to her. You hang tight here with grandma for a few minutes okay?" Jess nodded as she started to cry all over again.

Maura made her way into the guesthouse where she found her sister in equal hysterics. "Why don't you let me give you something to relax you Cailin? You're not in any shape to get yourself worked up like this."

"S-s-she l-l-lied to me Maura."

Maura nodded as she reached out and grasped her sister's hand in her own. "I understand Cailin. She lied to me also. She stole from me. I'm not saying she's not responsible for her own actions, she is. Her and I addressed that this morning. She knows she was wrong, but Cailin, we both know those hurtful things is not who Jess is. She made some horrible mistakes, but at some point we all have to give her the chance to prove to us she's going to put all this behind her. We can either choose to forgive and work towards healing everything that has happened, or we can hold a grudge and be bitter. The choice is up to you but I will tell you this.Though she is much better, Jess is still going through detox at this point. I'm having to keep a close eye on her. So if you want to stay here with me, you are going to have to put the bitterness and anger aside enough for you to both be here together. Can you do that?"

"I'm not making any promises, but I'll try." Cailin kept her eyes down as she continued to speak. "She lied to me Maura, she hid things. She cost me time with mom. What would have happened had that been the last time I spoke to mom? I want to be so angry with her. If Jess was anyone else I would break it off here and now."

Cailin wiped at the tears still falling and Maura took the moment of pause to speak.

"Cailin, I'm not asking you to act as if everything is okay. I'm just asking you to be civil because believe it or not you hold all the power right now. If you let your anger and hurt get the best of you, you could send her right back over the edge that she has fought so hard to pull herself back from."

Maura sighed and pulled her sister close as she began to sob."Cailin let me give you something to help you relax. You are too sick to be this worked up."

Cailin nodded before letting Maura up.

Maura walked across the room and dug through her bag, digging out the anxiety medication she had been giving Jess.

"I'm just going to give you the same anxiety med I've been giving Jess. It will help you relax and you'll get sleepy. Don't worry. I'll save you some sticky buns," Maura says in an attempt to get a smile from her sister. It worked.

"Mom's going to head over to the clinic and then the pharmacy after lunch so we can have everything on hand we need for you."

Maura drew up the meds and looked down at the I.V. in her sister's hand and started to laugh. "I still can't believe you walked all the way here pushing an I.V. pole Cailin," Maura says as she shakes her head before injecting the medication.

"You know we're going to have to return that right," Maura says with a grin.

Cailin however, this time didn't smile as she started to cry again.

"I was worried. I did it because I love her. I want to run so bad right now Maura. I'm scared. I may have the power to destroy her but she has the power to destroy me. I can't bear the thought of losing her."

Cailin's eyes started to flutter as the meds Maura had given her started to take effect. Maura noticing, gently guided Cailin to lay down. She sat on the edge of the bed and spoke softly as she stroked her sister's hair.

"You won't lose her Cailin, you two are like Yin and Yang. You need her as much as she needs you. The two of you remind me a lot of myself and Jane. It's going to take time but you will get through this together."

Cailin mumbled something Maura didn't quite understand before her breathing evened out signaling to Maura that she was asleep.

Doing her best not to wake her sister Maura slowly moved from the bed, making sure to cover Cailin before quietly making her way back outside.

Once outside Maura inhaled deeply. She was well aware of how rough the next few days or so would be but she hoped everything would work out for the best in the end. As she opened her eyes she saw Jess sitting with her mother. Deciding to give them a few minutes alone she made her way over and into the main house where Jane was standing in the kitchen.

Jane looked up from her cell as she heard the side door open and shut. Seeing her wife, she put her phone down and moved across the kitchen taking Maura into her arms.

Kissing the top of Maura's head she spoke. "That bad huh?" The blonde nodded.

"Things are going to be rough for Cailin and Jess for a bit. We will have to figure out new sleeping arrangements given that Cailin is not in the mood to be anywhere near Jess right now." Maura said all of this without lifting her head from Jane's chest.

"I'm sure we can figure something out Maur. Don't worry, everything will be okay," Jane remarked, giving her wife a gentle squeeze.

Maura wanted so badly to believe her wife's words, that everything would be okay moving forward but the way things had been going lately she wasn't so sure.

Hope walked in and interrupted the two ladies.

"Hey Maur. Jess wants you. I tried to help but she insists on mom," Hope says with a smile. "She's having a pretty intense craving. At least she is being honest with us. I am proud of her for that."

Maura nodded and kissed Jane before making her way back over to the guest house to her daughter.

As she walked in the sight of her daughter made her want to cry. The struggle of a heroin addict was heartbreaking. She had been great that morning but the events of the day had taken their toll.

She looked at Maura as she had tears running down her face. "I want it mom. You don't know how bad my body actually craves it. I just want to climb the walls right now."

"Try to relax. Remember what we have talked about. You have got past the hard part. This will pass. You are due for a dose of Buprenorphine. That will help curb the craving. I'll also give you something for anxiety to help you relax. You can do this Jess."

Maura went over to her bag and pulled out Jess's medication and took it to her after grabbing her a glass of water. Since Jess was doing better, Maura had taken out the IV and gave her strictly oral meds. After Jess had taken it, Maura looked over at Cailin then back to Jess.

"Why don't you go lay down in Adam's bed?"

Before Jess could reply, Cailin turned over in bed and looked groggily over at Maura and Jess.

The state of Jess startled Cailin. It was something like she had never seen before. Remembering Maura's earlier words, she decided though she was still upset, she and Jess would work through it in time. Right now, she knew she had to step up and calm the fears that were raging in Jess's mind. The last thing she wanted to do was push her over the edge and back into drugs.

Cailin patted the spot beside her.

"Come on baby. Lay down with me." Jess looked over at Cailin, silently asking if she was sure. Again, Cailin patted the empty spot.

Jess made her way over to the bed and laid down. Cailin had turned back onto her side facing away from the redhead. Jess laid flat on her back looking up at the ceiling. She wanted more than anything to hold her girlfriend but decided against it.

As if Cailin was reading Jess's thoughts she rolled onto her back, opening her arms for Jess. Without a second thought Jess moved into Cailin's arms.

When Cailin pulled her closer she began to cry. "I'm sorry baby, I'm so sorry." Before Cailin could say anything Jess started to sob.

Cailin started rubbing her back. "Sleep Jess, we can talk later." Jess immediately shook her head.

"I want it so bad C, so bad." Cailin's heart was breaking at her girlfriend's words. She was seeing and hearing the struggle and the pure vulnerability in every word.

Cailin tilted Jess's head up towards her. She kissed her lips softly before resting their heads together. "No Jess, you want an escape but I'm your escape. I'm your safe place." Cailin paused as she swallowed down her own emotions.

"I love you Jess.We will get past all of this with time, but for right now, just let me hold you. Let your body rest, I'm here, right here and I will be here when you wake up. Okay?"

Jess sniffled and nodded before snuggling in closer. It didn't take long before both girls were sound asleep.

Maura silently tip toed over and made sure both girls were covered well before slipping outside. She knew she should have stepped away while they were talking, but she stuck around in case anything headed south. She was relieved that maybe they were going to be OK after all.

Just as she stepped outside, the whole gang was heading her way, food in tow. Maura held up her hand. "They are both sleeping. It's a nice day. Why don't we all eat out here?"

Jane nodded.

"Can you and Ry help me move a couple of the picnic tables over here? I at least want to be nearby to check on them and in case they wake." Maura asked, not wanting to be to far from the girls.

Jane nodded. "Sure babe."

Jane and Riley made the necessary adjustments to keep the mama bear in Maura satisfied, before the family sat down for lunch.

As everyone was eating, Abby all of a sudden squealed when a butterfly landed on her and she did a somersault from the bench where she was sitting and began to scream.

Making her way to Jane, she gripped Jane's leg tightly as if the attack of the butterfly would return at any moment. Not wanting to make light of her daughter's fear, Jane worked extra hard not to laugh.

Maddy looked over at her sister with the most serious expression Jane had ever seen on a five year old. "It won't hurt you Abby." Maddy proceeded to explain in great detail the metamorphosis of a butterfly.

Everyone dropped their jaws in shock. Jane finally lost it as she began to laugh. "Maura, our five year old daughter just described metamorphosis. I know biologically she's mine, but I'm pretty sure that was one hundred percent you."

"Well she does look like me, maybe she is mine." Maura laughed when Jane choked on her water.

"That's not funny Maur."

"Love, I'm kidding. Young children can pick up the habits of their parents by watching. I'm sure Maddy has heard me explain in detail many things to you or she could be intrigued by butterflies and has learned as much as she could."

The women turned their attention to Hope when her phone started to ring and she stepped away to answer it.Maura groaned.

"She's taking calls in secret again. It better not be Paddy."

"I'm sure it's nothing Maur. She is a doctor. It could be a patient. You know, that whole patient confidentiality thing?"

Maura shrugged her shoulders. "I don't know. I never have to worry too much about that. Even if I broke my patients confidence, there is zero chance of them filing a complaint."

For the second time that day, Jane choked on her water, this time because she was laughing. "That was awful Maur, awful."

Suddenly Hope reappeared grinning ear to ear as she looked at Maura. "Your phone is about to ring."

As the words left Hope's mouth, Maura's cell began to ring.

She looked at her mother funny before stepping away herself to take the mysterious phone call.

Suddenly, Jane was wondering if Maura was right. "I swear Hope, if you gave Paddy her phone number…"

"Relax Jane. It's not Paddy. You'll see who it is in just a minute."

A couple minutes later, Maura reappeared also wearing a wide grin on her face. She quickly ran over to Hope and grabbed her mother in a hug as they both began to jump up and down in excitement, followed by tears of joy.

Maura turned to Jane and smiled."Okay everyone. We have an announcement to make." Maura turned to Hope, letting her do the honors.

"They have found a match for Cailin."

Hope paused for a moment to let that sink in with everyone before she continued. "It's Maddy. Maddy is a perfect match for Cailin!"

Soon everyone was celebrating, exchanging hugs, high fives, and happy tears.

For the second time that day, Maddy sat amongst all the commotion with a serious look on her face.

She shook her head from side to side. "Uh uh. I don't look like Cailin. You're being silly grandma. Mommy matches Aunt Kate," the five year old said matter of factly.

With that, everyone broke into laughter. Jane smiled at her daughter. She couldn't believe how much like Maura she was becoming as she got older.

For a fleeting moment Jane had a thought but just like that she pushed it away. "No way Jane. Don't be ridiculous," she mumbled to herself.

"What was that love?" Maura looked over and smiled at her wife.

"I said that's great. I can't wait to break the news to the girls."

Maura smiled."About that, I was thinking maybe we could have a small get together tomorrow to celebrate. Your family hasn't come in a while because of everything going on. Maybe they can join us. They will of course need to keep a good distance from Cailin under the circumstances, but I think we need to make the best of it and celebrate."

Jane nodded. "That sounds great Maur. They will love that. Maybe we can go have a t-shirt made for Maddy to wear to break the news."

"That's a wonderful idea Jane!" Maura grabbed a hold of her wife and hugged her tightly.

Later, after lunch was done and everyone had gone their separate ways, Jane was at the main house helping Hope put away the leftovers.

"Hope, isn't it pretty astronomical odds for Maddy being a match? With her being my biological daughter, I really didn't have a lot of hope but decided to have the twins tested anyway, along with myself of course."

Hope nodded. "It's extremely rare but it does happen. It's about the chance you would have of winning the lottery. One in a million odds. We won the jackpot."

Again, Jane had that thought and tried desperately to push it away. She smiled at Hope before making her way back out to the guest house where Maura was sitting under the shade of a tree reading to the twins and Amelia.

She looked from Maddy to Maura and back again several times. Her daughter did have a striking resemblance to Maura. "You're being ridiculous Jane," she again mumbled to herself.

"There's no way Maura would be unfaithful to me."

As she sat there she struggled with the thoughts she was having. She knew the only way to put her mind at ease would be to straight up ask. She knew Maura wasn't going to like it, but she couldn't shake this feeling in the pit of her stomach.

As much as she loved and wanted to trust Maura completely, her mind couldn't help but wonder back to years ago when Ashley had broken her heart.
