Regina was so upset and afraid that she had no idea what was going on. To her surprise, Mary Margaret did. "Regina," she said softly. The teacher sat in a chair by the mayor's bed and took her hand. "They need to take you into surgery."

"Is my baby going to die?" she asked through tears.

"I wish I could tell you that the baby was gonna be fine, but that's something I don't know."

"I'd rather you be honest."

"You've started to bleed more heavily. Dr. Whale is having trouble finding a heartbeat. He's hoping surgery can save the baby, but there's no guarantee."

"Will you wait with me until they take me back?"

"Yes, and we'll all be here when you wake up. If Emma's here, then we'll be in the waiting room."

Mary Margaret sat with the distraught mayor until they took her back to the OR. "How is she?" Granny asked.

"Devastated and afraid," the teacher answered as she took a seat.

"What did you tell her?" Mother Superior asked.

"She wanted the truth. I just told her they weren't sure the baby could be saved."

"You are the best one to break news like that," Granny said.

"I wish I wasn't. That was awful."

"Hey," Emma ran into the waiting room. "Where's Regina?"

"In surgery," Mary Margaret answered. "They can't find a heartbeat and she's starting to bleed more heavily. They don't think the baby can be saved, but she can be. I told her they were hoping to save the baby, but there was no guarantee. I also said we'd all be here when she woke up. Probably should have asked you all first."

"I'm not leaving," Granny said. The other two agreed.

While they were waiting, Graham walked in holding Henry and Remy, "look who I found," he said.

"Oh thank God," Emma rushed to her boys.

"How's Regina?" the sheriff asked.

"In surgery," Emma answered, "she should be fine, but..."

"Why don't I take the boys home and put them in bed?" he asked knowing where Emma was going with her answer.

"That would be great."

"I'll have Sister Astrid go over to help," Mother Superior said. Graham switched cars with Emma since she was driving one with the car seats. He left with the boys.

Regina was devastated to find out that her baby didn't make it. Emma did her best to comfort her girlfriend. Dr. Whale prescribed Regina some medication to help with the depression. "I was thinking," Emma said one night in bed, "why don't we go away for a week or two?"

"A vacation?"

"Yeah," Emma answered, "we could take the boys or go just the two of us."

"I don't know."

"Come on," Emma said, "what's the fun of being able to cross the town line if you never cross it?"

"I'll think about it."

"That's all I ask."

"Emma," Regina looked at her girlfriend.

"Yeah?"

"Thank you for everything. I don't know how I would have gotten through all of this without you."

"You don't have to thank me. It's what you do for the ones you love."

Regina smiled, "what would you say if I decided to try again?"

"I'm here for you no matter what."

"I don't know what I did to deserve you."

"Me either," Emma joked, "I mean, you told me your story and I gotta say, I'm way out of your league."

Regina laughed and kissed the blonde. Emma always knew how to make Regina smile or laugh. The blonde always knew what the mayor needed and when she needed it.

Due to the boys being so young, Emma and Regina decided to go away without them. Mary Margaret, Mother Superior, and some of the other nuns agreed to take care of the boys while their mothers were away. "Wow," Emma said when the couple entered their hut. "You really went all out."

"Well, the beach and ocean are calming."

"I gotta say, I really like being away with you. It's never been just the two of us."

"Well, a hut in the ocean is the perfect way to isolate ourselves."

"I've never heard of the Maldives, but it's now my favorite destination."

"Have you been outside of the states?"

"No, but I've been to many states and this is way better."

"Do you mind if I lie down?"

"Are you okay?"

"I think it's just jet lag." Regina stood there instead of walking away, "actually, could you help me?"

Emma rushed to take Regina's arm. It was concerning for Emma that Regina still needed her help even with the cane. Regina spent the rest of the day in bed. Emma didn't mind because she knew the woman needed the rest and they had plenty of time to enjoy their vacation.

Emma and Regina had a great time on vacation. They felt incredibly relaxed by the time they left. Regina had decided to try to get pregnant again. Emma supporter her, but time and time again the mayor was disappointed. Eventually, her doctor made her stop. It took time for Regina to be able to move on, but eventually, she did. Eight years after that first trip to the Maldives, the two women returned. Only this time, Emma had something else planned. When they walked into their hut, there were rose petals scattered.

"Wow," Regina said, "are we in the wrong hut?"

"Nope," Emma answered, "this is for you."

"You did this?"

"Well, I ordered it because obviously, I wasn't going to be here to do it."

"It's so beautiful."

That night at dinner, Emma made her move. She proposed to her girlfriend. Regina was shocked but happy. She accepted with great excitement. "I have to admit, I was worried you'd say no."

"Why?"

"I don't know. It's what I do."

"This entire trip was so you could propose?"

"Well, yeah," Emma answered. "I wanted this to be special."

Regina smiled and kissed her fiance. Things were starting to look up for the once Evil Queen and the orphan.
