If it happened after they conquered Ba Sing Se
Azula would initiate it. Zuko would be in complete disbelief at not only being finally allowed to go back home, but also getting to have his little sister all to himself. I can see him being extremely loving to her because of what she just did for him, and he'd make sure her first time is perfect since she's being kinder to him that night than she had ever been their entire lives. They'd wake up in each other's arms, and might do it again in the morning.
If it happened during their confrontation on The Bedroom Scene
It'd be a mix of anger, resentment, lust and genuine affection. They'd both be firm, yet still gentle with each other. Azula would probably not tell him she's a virgin since they'd be doing it out of pure lust and not love, which means she wouldn't want him to know how vulnerable she's making herself to him, but he'd still go slow just in case since she's still so young. Zuko wouldn't stay the night because "it's purely physical", but there'd be that lingering feeling between them that this meant much more to them than they are ready to admit.
If it happened after the party on Ember Island
They'd both be very vulnerable and melancholic considering everything that happened that night. They could do it on the beach or in their old house, and it'd be a form of catharsis for them since there's so much pent-up tension between them and they were forced to confront some feelings about their family they didn't want to acknowledge. They'd be very passionate, maybe leave a few small burns and love bites all over each other, spend all night up just seeking comfort in each other and wouldn't join their group again until the next day.
If it happened after Azula already had her breakdown
Zuko would constantly be reassuring her that he really does love her - he always says it to mean familial love, but he's starting to realize it's more than that he is soooo bad at dealing with it. In one of Azula's worst days, she's completely convinced she's fundamentally unlovable, and Zuko would just snap in the middle of his hundredth "I love you" speech, and kiss her. He'd be horrified at what he did and start apologizing, but Azula would just pull him close to her again and then hesitantly kiss him herself. They'd slowly get more and more desperate for each other, Azula craving all the love her brother has to give, and Zuko barely even believing said love is mutual.
If it were a modern AU
Once again, Azula would initiate it. She'd see Zuko as hers, and herself as his, and she'd not accept being told "It's wrong/inappropriate for siblings to love each other like that". She'd go to his room one night and just kiss him. Zuko, who would have been dealing with a lot of guilt and shame over his feelings, would try to tell her they shouldn't do this… only to succumb to her, because he absolutely loves his little sister and the thought of being her first and only lover is just too much for him to resist.
