First half of Book 3
He would be really freaked out and wouldn't leave her side. At first, Azula would assume he's just scared that he will be the next and won't manage to survive like she did, but she quickly realizes that, yes, that dumbass really does care about her and was worried he had lost her. She'd be nicer to him… and enjoy seeing how he's willingly doing literally anything she asks him to, no matter how ridiculous. Despite his obvious devotion, they'll both deny it if anyone so much as jokes about the fact that they really do love each other.
Second half of Book 3
He'd be very conflicted. He knows his sister is not a good person, he knows she'll keep helping their father as long as she lives, he knows she's insanely dangerous, he knows she wouldn't care if he died… but she's still his baby sister, and he just can't accept that she is hurt, that he wasn't there to protect her, that she didn't want him to be there to protect her. Caring about her makes him feel like an idiot, but not caring is just impossible for him.
After he became Fire Lord
He'd take her out of the hospital he had sent her to, bring her back to the palace, make sure she's guarded 24/7, hunt down the people responsible himself, and not accept anyone saying she doesn't deserve his help. He is the Fire Lord now, and he can do whatever the hell he wants - and what he wants to is to help his little sister. Even after she is fully safe again, he keeps her in the palace.
