Important: The "unrequited" aspect of it has to be temporary. It just has to be. If one of them crosses that line, it's only a matter of time for the other to do it too. I don't care if it takes a weeks, months, or years - it will happen.
If it's Zuko who has one-sided feelings for his sister…
He will 1.000.000% pine for her. He'll make sure to spend lots of time around her, even when she's getting on his nerves. His friends and other relatives are sick of hearing him talk about her, and want to slap him every time he insists on spending time with her even when she's being a bitch to him.
He will go ballistic at any guy who so much as looks at Azula, and God help the poor fucker if they don't have the most wholesome intentions with his little sister.
If Azula already her breakdown, Zuko will be unbelievably protective and defensive of her, to the point that the doctors want to forbid him from visiting her, but they can't because he's the fucking Fire Lord, so they just have to watch this disaster unfold.
Azula, as well as everyone around them, would figure it out because Zuko is physically incapable of being subtle. If she's still trying to be Ozai's little girl, then she'll either mock Zuko for it or try to use it to manipulate him. If she already had her breakdown, she'll actually be happy about it since it means she really IS loved, even if it's not in a way she expects or can reciprocate (yet).
If it's Azula who has one-sided feelings for her brother…
She would NOT handle it well. If it were just sexual attraction, she would have shameless tried to seduce him (*cough, cough* bedroom scene *cough*), but truly caring about him? That would terrify her, no matter at what point in their lives.
Her attempts at flirting would be disastrous, and no one would even realize that's what she's doing. I can see her oscillating between being super mean to him and being unbelievably nice, and Zuko would be confused as fuck.
Once she did actually confess her feelings to him Zuko would be shocked. If it happened before her breakdown, he would oscillate between avoiding her and trying to build a normal sibling relationship with her. If it happened after her breakdown, her vulnerability would make Zuko decide to be by her side all the time… after he screamed at the universe for just not letting ANYTHING in his family being normal.
She would be hurt that he didn't love her like that (yet), but eventually she'd just be happy that he didn't think less of her because of it (but only after her breakdown, because she's still proud).
However, eventually accepting his (temporary) rejection doesn't mean she'll be okay with seeing him with someone else. He'll date her, or he won't date at all. Good thing she'll have doctors checking on her, 'cause i can see her going full yandere if she doesn't get some therapy.
