The gist of it all is, Dean is gagged, blindfolded, and bound, with his knees spread open by a rope. Castiel has a vibrator in him that has ten settings, and Dean always finishes at the last one, except Cas keeps resetting it after 9, not letting Dean finish. He does this 4 times until Dean is a sobbing, begging mess.
