Minato during his life was known as Senju Tobirama's Grandson. His name was Senju Minato.
Naruto ends up with the surname Namikaze.
The sound of flesh ripping apart was ringing loud across the destroyed and wrecked battlefield. Sarutobi Hiruzen, Sandaime Hokage looked forward through the chain barrier created by the previous Kyubi Jinchuriki, Uzumaki Kushina, and saw herself as well as her husband and Hiruzen's successor, Yondaime Hokage, Senju Minato being skewered on one of the sharp nails of the Kyubi who was being held back by Kushina's chains.
Hiruzen glared at the barrier and attempted to break through with his adamantine staff which just got deflected. Two more jonin land beside him, "Sandaime-sama, what's wrong?!" one of them ask. Hiruzen then tried to pushed his hand through to no avail. "It's no good. I can't go further than this point because of the barrier put up to keep the Kyubi from escaping! Those two are planning to take care of the Kyubi themselves!" Hiruzen said in frustration.
Hiruzen then looked closer to them and realized when they got hit by the nail, they were protecting a baby. "There's a child." Hiruzen said. "They shielded him." another jonin said. They then saw Minato go through hand signs and then summoned a toad which had a body made out of a scroll. It then looked like Minato created a sealing formula on the toad's scroll, before the toad disappeared.
Hiruzen narrowed his eyes, "I see...Minato plans to make his own son the Jinchuriki to save the village." Hiruzen said in understanding. Hiruzen then heard Kushina beginning to speak to her child, and give her last words to her son in preparation of their separation. Kushina began to sob in sadness, as Minato gave his couple words. Minato then went through a couple hands signs, "Hakke fuin..." he said quietly.
Suddenly the last half of the Kyubi became converted into the form of chakra and began to funnel into a seal which appeared on the child's stomach. Due to the Kyubi not being in physical form no longer, both Kushina and Minato were not skewered on the nail and fell down to the ground. Minato had instantly died, while Kushina was barely clinging to life.
The chains began to disappear and the barrier came down, prompting Hiruzen and the two Jonin with him to rush to their side. Hiruzen picked up the child from the sealing altar and observed the baby. The baby was a boy, with a fair skin complexion, and had spiky dark red hair. The seal of the Kyubi and whisker marks, showing his proof of being a jinchuriki. Suddenly the seal faded from sight, as well as the whisker marks.
Hiruzen looked down at the child in concern. One of the other Jonin checked on Minato, "Yondaime!" he said in distraught over his death. The other jonin helped Kushina sit up, "Kushina-sama!" he said in worry. Hiruzen looked at her injured state, "Take her to the Leaf Hospital immediately! Summon the Medical Ninja!" Hiruzen ordered. Kushina opened her eyes weakly and looked at Hiruzen, "Sandaime..." she said weakly. Hiruzen looked at her in worry and quickly came to his knees next to her.
He looked at her eyes started dull. "Kushina!" he said with concern. "Please take care...of the child." she requested. "Of course, you don't need to worry." Hiruzen told her. "His name is...Naruto...Minato and I...picked the name from Jiraiya-Sensei's novel..." Kushina said. Hiruzen nodded, "Naruto, you say? Fine. I will take full responsibility. So, for the child's sake, you must rest!" Hiruzen told while looking down at Naruto. He then realized that she never responded and looked back up and realized she had died. "Kushina! I promise..." Hiruzen said with sadness.
