John did think of them as a couple, just not in the same sense that everyone else seemed to think of them as a couple. More than friends in the way that simple friendship didn't explain everything that was Sherlock-and-John, but not in a consummated, romantic sort of way. It was just easier to say 'flatmate' than explain properly.
