Okay, so we're at the point where she's about to shoot lightning at him, Zuko is fully prepared to redirect it, and he is trying to ignore the part of him that is still desperately trying to find a way out of this situation, not just so he won't have to kill his sister, but because he can't help but want to save her.
But then Azula just… stops. The lightning she was generating was suddenly gone, and she's shaking while laughing in a really unnerving way… and she's on her knees, crying like a little kid.
Zuko would instantly freeze in place due to the absolute shock, then wonder if this is just Azula's last attempt at manipulating him to get the upper hand… but he'd decide to risk it and go to her, giving her a comforting hug as she clings onto him as if her life depended on it.
He hears whispering that she loves him, that she has always loved him, and at first he assumes she means it in the familial way… then she says she always wanted to be with him and kisses him. He is too stunned to react in any way.
Once he comes back two his senses, he'd help her heal. As for the romance, there are two possibilities:
1 - He is denial about his feelings for her/thinks acting on his feelings would be wrong, and spends months trying to fight it before giving in.
2 - He already knows well that he loves her and doesn't care about what's "right" or "wrong" and immediately promises Azula that he has always been and always will be hers.
