After Ichigo defeated Aizen, he lost his power and Aizen regenerated to obtain a new form. Kisuke was still conscious means Aizen was never sealed away.

"It also seems like I'm evolving again" smirk by Aizen.

Chad, Rukia, Uryu and Renji all attacking Aizen while Orihime trying to heal Ichigo but was futile. He couldn't even come back too and Orihime start to cry. Aizen kill everybody but Orhime and left her there crying.
Aizen: "How sad of her and now I think it is time to kill everybody in Kakakura town and all the soul reapers too" as he is evolving. All soul reapers even the surviving arrancers felt the reiatsu enormous which all feared them.
